运动负荷超声心动图(ESE)联合运动心电图(ExECG)诊断严重冠状动脉病变价值研究

摘　　要：目的:探讨运动负荷超声心动图(ESE)联合运动心电图(ExECG)诊断严重冠状动脉病变的临床价值。方法:选取2019年8月~2020年3月本院心内科收治的疑似冠状动脉病变的168例患者为研究对象,对其进行运动负荷超声心动图(ESE)联合运动心电图(ExECG)检查,并且在该检查1个月后对168例患者进行冠脉造影(CAG)或冠状动脉CT血管造影(CTA)检查。结果:运动负荷超声心动图(ESE)诊断的敏感度、特异度及准确性分别为77.63%、82.13%、80.56%。运动心电图(ExECG)诊断的敏感度、特异度及准确性分别为63.46%、60.89%、65.13%。运动负荷超声心动图(ESE)联合运动心电图(ExECG)双阳性诊断严重冠状动脉病变的阳性预测值为79.63%,双阴性的阴性预测值为93.48%。结论:运动负荷超声心动图(ESE)联合运动心电图(ExECG)诊断严重冠状动脉病变具有较高的临床应用价值。Objective:To explore the clinical value of exercise stress echocardiography(ESE)combined with exercise electrocardiography(ExECG)in the diagnosis of severe coronary artery disease.Methods:A total of 168 patients with suspected coronary artery disease admitted to the Department of Cardiology in our hospital from August 2019 to March 2020 were selected as the research subjects,and exercise stress echocardiography(ESE)combined with exercise electrocardiography(ExECG)was performed.One month later,180 patients underwent coronary angiography(CAG)or coronary CT angiography(CTA).Results:The sensitivity,specificity and accuracy of the diagnosis of exercise stress echocardiography(ESE)were 77.63%,82.13%and 80.56%,respectively.The sensitivity,specificity and accuracy of the diagnosis of exercise electrocardiogram(ExECG)were 63.46%,60.89%and 65.13%,respectively.Exercise stress echocardiography(ESE)combined with exercise electrocardiogram(ExECG)double positive diagnosis of severe coronary artery disease has a positive predictive value of 79.63%,and double negative negative predictive value is 93.48%.Conclusion:Exercise stress echocardiography(ESE)combined exercise Electrocardiogram(ExECG)diagnosis of severe coronary artery disease has high clinical application value and can be promoted clinically.
