二仙逍遥散治疗血液透析患者并发抑郁症肾虚肝郁型疗效观察

摘　　要：目的:观察二仙逍遥散对维持性血液透析并发抑郁症、中医辨证属肾虚肝郁证患者的临床疗效以及对这些患者微炎症的影响。方法:选取在兰州大学第一医院、天水市第一人民医院、天水市中西医结合医院血液净化中心接受透析治疗的130例终末期肾脏病行血液透析合并抑郁症、且中医辨证属肾虚肝郁证的患者,所有患者均给于基础疾病的常规治疗,随机数字表法将患者分为2组,对照组给予帕罗西汀治疗,观察组给予二仙逍遥散口服,疗程为8周,以汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)评定抑郁积分以及治疗前后减分率;监测患者治疗前后超敏C反应蛋白(hypersensitive c-reactive protein,hs-CRP)、白介素-6(interleukin-6,IL-6)、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)的变化。结果:治疗后,在第4周、第8周2组 HAMD积分均显著低于治疗前(均P<0.05),且观察组显著低于对照组(均P<0.05);治疗组总有效率明显高于对照组;治疗组hs-CRP、IL-6、TNF-α水平显著低于治疗前(均P<0.05)。结论:二仙逍遥散治疗维持性血液透析并发抑郁症、中医辨证属肾虚肝郁的患者,能减轻抑郁症状,改善微炎症状态。Objective:To observe the clinical efficacy of Erxian Xiaoyao San on maintenance hemodialysis patients with depression and TCM syndromes of kidney deficiency and liver depression,and its influence on microinflammation in these patients.Methods:Select 130 patients with end-stage renal disease who received dialysis treatment in the First Hospital of Lanzhou University,the First People’s Hospital of Tianshui City,and the Blood Purification Center of Tianshui City Integrated Traditional Chinese and Western Medicine Hospital.All patients were given conventional treatment for the underlying disease and were randomly divided into 2 groups.The control group was given paroxetine,and the observation group was given Erxian Xiaoyao San.The course of treatment was 8 weeks.The Hamilton Depression Scale (Hamilton Depression Scale,HAMD) to assess depression scores and rate of reduction before and after treatment;to monitor patients before and after treatment for hypersensitive c-reactive protein (hs-CRP),interleukin-6 (IL-6),tumor necrosis Factor α (tumor necrosis factor-α,TNF-α) changes.Results:After treatment,in the 4th and 8th week,the HAMD scores of the 2 groups were significantly lower than before treatment (P<0.05),and the observation group was significantly lower than the control group (P<0.05);the total effective rate of the treatment group Significantly higher than the control group;the levels of hs-CRP,IL-6,and TNF-α in the treatment group were significantly lower than before treatment (P<0.05).Conclusion:Erxian Xiaoyao Powder can treat maintenance hemodialysis patients with depression and TCM syndromes of kidney deficiency and liver depression,which can relieve depression symptoms and improve micro-inflammatory state.
