中药骨痹通消颗粒联合髓芯减压植骨术治疗ARCOⅠ-Ⅱ期股骨头坏死临床疗效观察

摘　　要：目的:观察中药骨痹通消颗粒联合髓芯减压植骨术治疗ARCOⅠ-Ⅱ期股骨头坏死的临床疗效。方法:将2017年 10月-2019年 7月收治的48例(50髋)ARCOⅠ-Ⅱ期股骨头坏死患者分为治疗组和对照组。其中治疗组24例(25髋),治疗组在髓芯减压植骨术的基础上配合院内制剂骨痹通消颗粒口服,口服时间为6个月;对照组24例(25髋)仅做髓芯减压植骨术处理。术后采用髋关节Harris 评分、疼痛视觉模拟评分(visual analogue scale,VAS)进行两组患者疗效评价。结果:术后所有患者切口愈合良好,无术后感染、血栓等严重并发症发生,并均获得随访。治疗组术后3月、6月及12月髋关节Harris评分以及VAS评分均优于对照组,差异有统计学意义(P<0.05)。结论:中药骨痹通消颗粒口服联合髓芯减压植骨术能够在短期内改善髋关节功能,延缓早起股骨头坏死进一步发展。Objective:To observe the clinical effect of Gubi Tongxiao Granule combined with marrow core decompression and bone grafting in the treatment of ARCO Ⅰ-Ⅱ femoral necrosis.Methods:48 patients (50 hips) with ARCO Ⅰ-Ⅱ femoral head necrosis from October 2017 to July 2019 were divided into treatment group and control group.Among them,24 patients (25 hips) in the treatment group were taken orally for 6 months on the basis of core decompression and bone grafting in combination with the hospital preparation Gubi tongxiao Granules.In the control group,24 patients (25 hips) were treated with core decompression and bone grafting.Postoperative Harris score and Visual analogue Scale (VAS) were used to evaluate the efficacy of the two groups of patients.Results:Incision healing was good in all patients,and no serious complications such as postoperative infection and thrombosis occurred,and all patients were followed up.Harris score and VAS score of hip joint of the treatment group were better than those of the control group 3 months,6 months and 12 months after surgery,and the difference was statistically significant (P<0.05).Conclusion:Oral administration of Gubi Tongxiao Granules combined with bone marrow decompression and bone grafting can improve hip joint function in a short term and delay the further development of femoral head necrosis in early stage.
