摘　　要：目的探讨复种BCG或结核菌素试验复强反应对外周血CD4^(+)记忆T细胞特征的影响。方法将结核菌素纯蛋白衍生物(TB-PPD)皮试和干扰素释放试验(QuantiFERON-TB Gold in-tube,QFT)均为阴性的健康者(PPD-QFT-)分成2组,其中一组接种BCG(BCG复种组),另一组接种安慰剂(对照组)。12周后,采集受试者外周血进行流式细胞术分析,然后在受试者右臂和左臂皮内分别注射TB-PPD和重组结核杆菌融合蛋白ESAT6-CFP10(EC),48 h观察皮试反应。结果对照组和BCG复种组再次TB-PPD皮试阳性率分别为14.3%和47.9%。对照组、TB-PPD阳转者与TB-PPD阴性反应者比较,其外周血CD27-CD28+CD4^(+)-EM细胞百分含量增加而CD27+CD28-CD4^(+)-CM细胞百分含量降低。BCG复种组、TB-PPD阳转者外周血CD27+CD28-CD4^(+)-EM细胞含量低于TB-PPD阴性反应者。结论成人复种BCG或TB-PPD皮试复强反应可影响外周血CD4^(+)The object of this study was to investigate the effects of how BCG revaccination or boosting of tuberculin reactions on the characteristics of peripheral blood CD4^(+)memory T cells.All participants were subject to skin tests of purified protein derivative of tuberculin(TB-PPD)and an interferon-γrelease assay(QuantiFERON-TB Gold in-tube,QFT)at the same time;both TB-PPD-and QFT-negative participants were assigned to BCG revaccination(BCG revaccination group)or placebo(control group)in a 1:1 ratio.After 12 weeks of treatment,the percentage of peripheral blood CD4^(+)T-cell subsets was analyzed with flow cytometry,and then tuberculin reactions were read at 48 h after TB-PPD and recombinant fusion protein ESAT6-CFP10(EC)given concomitantly on forearms using the Mantoux technique.Data showed that 14.3%of the control group and 47.9%of the BCG revaccination group demonstrated TB-PPD conversion of positive.For the control group,the percentage of CD27-CD28+CD4^(+)-EM was increased,while CD27+CD28-CD4^(+)-CM decreased in TB-PPD positive conversion participants,as compared with TB-PPD negative participants.For the BCG revaccination group,the percentage of CD27+CD28-CD4^(+)-EM in peripheral blood from participants with positive reaction to TB-PPD was lower than that from participants with negative reaction to TB-PPD.Our results suggest that BCG revaccination or boosting of TB-PPD skin tests might influence the expression of CD27 and CD28 on CD4^(+)memory T cells.
