老年COPD患者胸部CT扫描测定肺/主动脉直径比值与其肺功能的关系探究

摘　　要：目的探究老年慢性阻塞性肺疾病(COPD)患者胸部电子计算机断层扫描(CT)测定肺动脉直径(PA)/主动脉(A)直径比值与其肺功能的关系。方法选取我院2016年1月到2019年1月确诊为COPD患者78例,并按照COPD病变标准分为4个亚组,均通过CT测定PA、A,计算PA∶A比值,并分析其与COPD分级及肺功能指标的相关性。结果随着COPD病症分级的增加,患者PA∶A比值呈逐渐上升趋势,各亚组之间差异有统计学意义(P<0.05);同时,秒用力呼气容积(FEV1)、用力肺活量(FVC)一秒率(FEV1/FVC)、一秒用力呼吸容积占预计值比例(FEV1%)、残气量/肺总量(RV/TLC)均依次下降,各亚组之间差异比较有统计学意义(P<0.05);COPD患者PA∶A值与COPD分级呈显著正相关(P<0.05),与肺功能指标FEV1、FVC、FEV1/FVC、FEV1%、RV/TLC显著负相关(P<0.05)。结论老年COPD患者经胸部CT测得的PA∶A值与肺功能显著相关,可作为评估COPD病情的临床参考指标。Objective To explore the relationship between the pulmonary artery(PA)/aorta(A)diameter measured by chest computed tomography(CT)and pulmonary function in elderly patients with chronic obstructive pulmonary disease(COPD).Methods 78 patients confirmed with COPD in the hospital from January 2016 to January 2019 were enrolled.According to the criteria of COPD lesions,they were divided into 4 subgroups.PA and A were measured by CT.PA/A was calculated.And its correlation with COPD grading and pulmonary function indexes was analyzed.Results With the increase of COPD grading,PA/A was increased gradually.The differences among all groups were statistically significant(P<0.05).On the other hand,forced expiratory volume in one second(FEV1),FEV1/forced vital capacity(FVC)(FEV1/FVC),FEV1 as a percentage of predicted value(FEV1%),and residual volume/total lung capacity(RV/TLC)were decreased.The differences among all groups were statistically significant(P<0.05).PA/A was significantly positively correlated with COPD grading in COPD patients(P<0.05),while significantly negatively correlated with FEV1,FVC,FEV1/FVC,FEV1%,and RV/TLC(P<0.05).Conclusion PA/A is significantly correlated with pulmonary function in elderly COPD patients,which can be applied as a clinical reference index for COPD diagnosis.
