儿童股骨近端肿瘤及肿瘤样病变CT影像学表现及临床价值分析

摘　　要：目的分析儿童股骨近端肿瘤及肿瘤样病变CT影像学表现及临床价值。方法回顾性分析本院2017年3月至2019年10月收治的49例股骨近端骨肿瘤和肿瘤样病变患儿的临床资料,分析病变的影像学特征及CT检查对患儿骨肿瘤和肿瘤样病变的诊断准确率。结果经CT检查股骨近端骨肿瘤和肿瘤样病变的诊断准确率为90.30%。3例骨样骨瘤中CT可见骨内软组织透亮区,周围可见硬化环。5例骨软骨瘤中,3例肿瘤与受累骨皮质和骨松质相连,软骨部分呈软组织低密度,2例可见不规则的钙化及骨化。6例骨巨细胞瘤中,4例可见纤细骨嵴。1例软骨肉瘤伴骨外软组织肿块,且肿块较大,钙化常见。3例恶性骨肉瘤中,1例可见日光样骨膜反应,2例可见放射状骨膜反应。13例单纯性骨囊肿CT可见不规则的溶骨性破坏,病灶内为均匀的液体密度影,18例动脉瘤样骨囊肿中9例病变内可见液-液平面。结论CT检查可有效显示儿童股骨近端骨肿瘤与肿瘤样病变的影像学特点,诊断准确率高,具有较高的临床诊断价值。Objective To analyze the CT imaging features and clinical value of proximal femoral bone tumors and tumor-like lesions in children.Methods The clinical data of 49 patients with proximal femoral bone tumors and tumor-like lesions admitted to our hospital from March 2017 to October 2019 were retrospectively analyzed.The imaging features of the lesions and diagnostic accuracy of CT examinations for proximal femoral bone tumors and tumor-like lesions of children were analyzed.Results The diagnostic accuracy of CT examination for proximal femoral bone tumors and tumor-like lesions in children was 90.30%.In 3 cases with osteoid osteoma,CT showed an endosseous translucent area of soft tissue,and a hardening ring was seen around it.In 5 cases with osteochondroma,3 cases were connected with the involved cortical bone and cancellous bone.The cartilage part showed soft tissue low density,and 2 cases showed irregular calcification and ossification.In 6 cases with giant cell tumor of bone,4 cases showed slender bone crest.One case with chondrosarcoma was accompanied by soft tissue mass outside the bone,and the mass was large,and calcification was common.In 3 cases of malignant osteosarcoma,1 case showed a solar-like periosteal reaction,and 2 cases showed a radial periosteal reaction.CT of 13 cases with simple bone cyst showed irregular osteolytic destruction,and uniform fluid density shadow was observed in the lesion.In the 18 aneurysmal bone cysts,9 cases can be seen with a liquid-liquid plane in the lesion.Conclusion CT examination can effectively show the imaging features of proximal femoral bone tumors and tumor-like lesions in children,and it has high diagnostic accuracy and high clinical diagnostic value.
