摘 要:缺血性脑卒中是危害人类健康的常见病、多发病,因其高发病率、高致残率和高病死率,给家庭、社会带来沉重的负担。脑组织经过一定时间的缺血后,再次恢复血液灌注时,其功能及结构损伤反而会加重,即脑缺血再灌注(I/R)损伤。目前有研究表明肢体远端缺血后处理(RIPostC)对I/R损伤后的脑组织具有内源性神经保护作用。大量的基础研究及临床研究显示,RIPostC对减轻I/R损伤和改善预后有重要作用。RIPostC主要通过神经、体液、免疫等多种途径在脑梗死发病过程的多个环节中发挥脑保护作用,但其确切机制尚不明确。本文就RIPostC脑保护机制的研究进展作一综述,旨在为RIPostC的临床转化及进一步的机制研究提供一定的参考。Ischemic stroke is a common and frequently-occurring disease,which is harmful to human health.It brings heavy burden to the family and society because of its high morbidity,high disability rate and high fatality rate.After a certain period of ischemia,the functional and structural damage of brain tissue will be aggravated when the blood perfusion is restored again,that is,cerebral ischemia-reperfusion(I/R)injury.At present,some studies have shown that remote limb ischemic post-conditioning(RIPostC)has an endogenous neuroprotective effect on the brain tissue after I/R injury.A large number of basic studies and clinical studies have shown that RIPostC plays an important role in alleviating I/R injury and improving prognosis.RIPostC plays a cerebral protection role in multiple links of the pathogenesis of cerebral infarction mainly through nerve,body fluids,immune and other ways,but its exact mechanism is still unclear.This article reviews the research progress of the cerebral protection mechanism of RIPostC,in order to provide some reference for the clinical transformation and further research of the mechanism of RIPostC.
