山楂降脂丸联合耳穴压豆疗法治疗H型高血压患者的临床效果及其对肾素-血管紧张素-醛固酮系统、颈动脉粥样硬化指标的影响

摘　　要：目的探讨在一般治疗的基础上采用山楂降脂丸联合耳穴压豆疗法治疗H型高血压患者的临床效果,以及对肾素-血管紧张素-醛固酮系统、颈动脉粥样硬化指标的影响。方法将60例H型高血压患者,随机分为对照组(n=30例)和观察组(n=30例)。所有患者均应用非洛地平片、马来酸依那普利叶酸片治疗,观察组在此基础上服用山楂降脂丸并行耳穴压豆疗法。比较两组患者中医疗效,以及治疗前后的血压、血浆同型半胱氨酸(Hcy)水平、血清醛固酮和促肾上腺皮质激素(ACTH)水平、肱踝脉搏波传导速度(baPWV)、颈动脉内膜-中膜厚度(cIMT)。结果1个疗程后,观察组的总有效率为90.00%,高于对照组的63.33%(P<0.05);两组血压、血浆Hcy水平、血清ACTH水平、cIMT、baPWV均降低,观察组血清醛固酮降低,且观察组以上指标均低于对照组(均P<0.05)。结论与单纯应用传统西药治疗相比,加服山楂降脂丸联合耳穴压豆疗法治疗H型高血压患者效果更为显著,可更有效地下调患者血压及Hcy水平,以及调控肾素-血管紧张素-醛固酮系统,更有利于防治颈动脉粥样硬化。Objective To explore the clinical effect of Shanzha Jiangzhi Pills combined with auricular point pressing with bean therapy based on conventional therapy on patients with H-type hypertension,and its impact on renin-angiotensin-aldosterone system and carotid atherosclerosis indices.Methods Sixty patients with H-type hypertension were randomly divided into control group(n=30)and observation group(n=30).All patients were treated with Felodipine Tablets and Enalapril Maleate and Folic Acid Tablets,based on which the observation group received Shanzha Jiangzhi Pills and auricular point pressing with bean therapy.A comparison was performed between the two groups in terms of efficacy of traditional Chinese medicine,as well as pre-and post-treatment blood pressure,plasma homocysteine(Hcy)level,serum aldosterone and adrenocorticotropic hormone(ACTH)levels,brachial-ankle pulse wave velocity(baPWV),and carotid intima-media thickness(cIMT).Results After one course of treatment,the total effective rate in the observation group was higher than that in the control group(90.00%vs 63.33%,P<0.05);blood pressure,plasma Hcy level,serum ACTH level,cIMT and baPWV were decreased in both groups,serum aldosterone was decreased in the observation group,and the observation group yielded lower indices mentioned above as compared with the control group(all P<0.05).Conclusion Compared with conventional western medicine therapy alone,its combination with Shanzha Jiangzhi Pills plus auricular point pressing with bean therapy exerts a more remarkable efficacy for patients with H-type hypertension,which can down-regulate patients′blood pressure and Hcy level more effectively,modulate renin-angiotensin-aldosterone system,and preferably contribute to prevention and treatment of carotid atherosclerosis.
