茵陈五苓散在抗结核药物性肝损害患者干预中的应用效果研究

摘　　要：目的探讨茵陈五苓散在抗结核药物性肝损害患者干预中的应用效果研究。方法符合纳入标准的2019年11月至2021年3月上海市浦东新区肺科医院抗结核药物性肝损坏患者98例,随机分为对照组(49例)和观察组(49例)。对照组在抗结核病药物的基础上给予肝泰乐注射液;观察组在对照组的基础上加用茵陈五苓散内服。2组疗程为4周。比较2组肝功能指标改善、临床疗效以及不良反应。结果治疗4周后,2组天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、γ-谷氨酰转移酶(GGT)、总胆红素(TBIL)明显降低,且观察组明显低于对照组(P<0.01);观察组治疗后的总有效率明显高于对照组(P<0.05);2组仅见轻度胃肠不适,差异无统计学意义(P>0.05)。结论在西医常规基础上应用茵陈五苓散干预抗结核药物性肝损害患者的疗效明显,能提高治疗效果,且安全性好。Objective To investigate the efficacy of YinchenWuling san on patients with liver injury induced by anti-tuberculosis drugs.Methods A total of 98 patients with anti-tuberculous drug-induced liver damage admitted to Pulmonary Hospital of Shanghai Pudong District from November 2019 to March 2021 were divided into control group(n=49)and observation group(n=49)by random number table method.The patients in control group were given with Gantaile injection on the basis of anti-tuberculosis drugs,and the ones in observation group were added with YinchenWuling san in addition.The improvement of liver function indexes,clinical efficacy,and the adverse reactions were compared between both groups.Results After 4 weeks treatment,aspartate aminotransferase(AST),alanine aminotransferase(ALT),γ-glutamyltransferase(GGT),total bilirubin(TBIL)were significantly decreased in two groups,and these indexes in observation group were significantly lower than that in control group(P<0.01).After treatment for 4 weeks,the total effective rate of observation group was significantly higher than that in control group(P<0.05).Only mild gastrointestinal discomfort was observed in the two groups,and the difference between the two groups was not statistically significant(P>0.05).Conclusion On the basis of the western medicine,YinchenWuling san has an obvious curative effect and has a good safety in the intervention of anti-tuberculosis drug-induced liver injury patients.
