右心衰、结核及肿瘤性患者胸腔积液中性粒细胞内p38蛋白对IFN-γ及TNF-α表达的影响及意义

摘　　要：目的观察右心衰、结核及肿瘤患者胸腔积液内中性粒细胞p38蛋白对IFN-γ及TNF-α表达的影响,探寻胸腔积液内中性粒细胞分布差异的原因。方法选取71例胸腔积液患者,收集其胸腔积液内中性粒细胞。根据胸腔积液内中性粒细胞的患者病因来源,分为心衰组(HF组)、结核组(TB组)、恶性组(CA组);加入SB203580处理的结核组,记为TB+SB203580组;加入SB203580处理的恶性组,记为CA+SB203580组。收集同期健康体检20人外周血并收集其内中性粒细胞,记为对照组。裂解以上各组中性粒细胞,采用Realtime PCR检测各组IFN-γmRNA、TNF-αmRNA表达水平;Western-blot检测各组p38蛋白、磷酸化p38蛋白、IFN-γ及TNF-α表达水平。结果各组p38蛋白表达水平与对照组相比,差异无统计学意义(P>0.05),其他各组磷酸化p38蛋白表达与对照组相比,差异有统计学意义(P<0.05);各组IFN-γmRNA、TNF-αmRNA表达水平与对照组相比,差异均有统计学意义(P<0.05)。结论右心衰、结核性以及恶性胸腔积液内中性粒细胞中的磷酸化p38蛋白升高,可导致中性粒细胞内IFN-γ和TNF-α在转录及蛋白表达水平升高,而SB203580通过抑制p38蛋白磷酸化降低结核组及肿瘤组的IFN-γ和TNF-α的表达水平,这种机制参与了中性粒细胞在不同病因的胸腔积液内的分布差异。Objective To observe the effect of p38 protein of neutrophils on the expression of IFN-γand TNF-αin pleural effusion of patients with right heart failure,tuberculosis and tumor,and to explore the reasons for the different distribution of neutrophils in pleural effusion.Methods The neutrophils were collected from the pleural effusion of 71 patients with pleural effusion.According to the etiology of neutrophils in pleural effusion,patients were divided into heart failure group(HF group),tuberculosis group(TB group)and tumor group(CA Group).TB group treated with SB203580 was recorded as TB+SB203580 group,and tumor group treated with SB203580 was recorded as Ca+SB203580 group.The neutrophils of 20 healthy adults in peripheral blood were collected as control group.The expression levels of IFN-γmRNA and TNF-αmRNA were detected by Real-time PCR.The expression levels of p38 protein,phosphorylated p38 protein,IFN-γand TNF-αwere detected by Western blotting.Results There was significant difference in the expression of phosphorylated p38 protein in other groups(P<0.05).Compared with the control group,the expression levels of IFN-γmRNA and TNF-αmRNA in the three group were statistically significant(P<0.05),and the expression levels of IFN-γand TNF-αin each group were statistically significant(P<0.05).Conclusion The increase of phosphorylated p38 protein in neutrophils in right heart failure,tuberculous pleural effusion and tumor pleural effusion can increase the levels of IFN-γand TNF-α,while SB203580 can reduce the levels of IFN-γand TNF-αby inhibiting the phosphorylation of p38 protein.This mechanism is involved in the differential distribution of neutrophils in pleural effusion of different etiologies.
