髓芯减压术联合骨诱导活性材料治疗早期股骨头坏死效果观察

摘　　要：目的:探讨髓芯减压术联合骨诱导活性材料治疗早期股骨头坏死的效果。方法:选取2017年1月至2020年1月临床上收治的60例早期股骨头患者分为观察组和对照组。对照组给予髓芯减压术治疗,观察组联合骨诱导活性材料治疗,比较两组患者髋关节功能恢复情况,治疗前、治疗后3个月及末次随访VAS评分、Harris评分、ONFH体积变化。结果:观察组患者末次随访髋关节优良率为93.33%,较对照组患者的73.33%显著提高(P<0.05);治疗前两组VAS、Harris评分无显著性差异(P>0.05),治疗后3个月、末次随访观察组患者VAS评分较对照组患者显著降低(P<0.05),Harris评分较对照组患者显著提高(P<0.05);治疗前两组患者ONFH体积无显著性差异(P>0.05),治疗后3个月、末次随访观察组患者ONFH体积较对照组患者均显著减小(P<0.05)。结论:髓芯减压术联合骨诱导活性材料治疗早期股骨头坏死可缓解患者疼痛,缩小股骨头坏死体积,促进其髋关节功能恢复,值得推广。Objective:To explore and analyze the effect of core decompression combined with osteoinductive active materials in the treatment of early femoral head necrosis.Methods:A total of 60 patients with early femoral head admitted from January 2017 to January 2020 were divided into observation group and control group.The control group was treated with core decompression,and the observation group was treated with osteoinduction active material.The recovery of hip joint function,VAS score,Harris score and ONFH volume were compared between the two groups before treatment,3 months after treatment and at the last follow-up.Results:The excellent and good rate of hip joint in the observation group at the last follow-up was 93.33%,which was significantly higher than that in the control group(73.33%,P<0.05).There were no significant differences in VAS and Harris scores between the two groups before treatment(P>0.05).At 3 months after treatment and the last follow-up,the VAS score in the observation group was significantly lower than that in the control group(P<0.05),while the Harris score was significantly higher than that in the control group(P<0.05).There was no significant difference in the volume of ONFH between the two groups before treatment(P>0.05),and the volume of ONFH in the observation group was significantly decreased compared with the control group 3 months after treatment and at the last follow-up(P<0.05).Conclusion:In the treatment of early necrosis of the femoral head,core decompression combined with osteoinduction active materials can relieve pain,reduce the volume of necrosis of the femoral head,and promote the recovery of hip function,which is worthy of promotion.
