摘　　要：目的探讨云南省玉溪市男男性行为者(MSM)与异性性行为时安全套使用情况及影响因素。方法根据《全国艾滋病哨点监测方案》要求,在2010-2019年以每年4-7月为监测期,采用横断面调查法对MSM人群的人口学特征、艾滋病知识知晓情况和行为学等连续监测,应用累计比数Logit模型分析MSM人群与异性性行为时安全套使用的影响因素。结果878名MSM最近6个月有异性性行为,其中18.79%(165/878)与异性性行为时从未使用安全套,30.52%(268/878)偶尔使用,50.68%(445/878)坚持使用。多因素分析结果显示,年龄越大、已婚/同居、户籍为外省、最近半年与同性发生过无保护性肛交和吸毒的MSM与异性性行为时更倾向于不使用安全套(OR>1,P<0.05);最近半年发生过商业同性性行为的MSM与异性性行为时更倾向于使用安全套(OR=0.551,95%CI:0.328~0.925)。结论MSM人群存在双性性行为,且发生异性性行为时安全套使用率较低,需根据影响MSM与异性性行为时安全套的因素制定相关干预措施,避免MSM人群中的性传播疾病向一般人群传播。Objective To explore the status of condoms use with female and its influencing factors among men who have sex with men(MSM)in Yuxi prefecture,Yunnan province.Methods According to the requirement of sentinel surveillance protocol,each year,cross-sectional survey were conducted to collect the information about the demographic characteristic,awareness of AIDS,and related behaviors between April and July from 2010 to 2019,the cumulative odds logit model was used to analyze the factors associated with condoms use among MSM had sex with female.Results Of 878 MSM had sex with female in the last six months,165(18.79%)never used condoms,while 30.52%and 50.68%were occasional and consistent used condoms respectively.The multivariate analyses indicated that the older,those married or cohabited,those come from other provinces,being had unprotected anal sex with men in the last six months and drug user tended not to use condoms with female(OR>1,P<0.05),while those had sex with money boy in the last six months tended to use condoms(OR=0.551,95%CI:0.328~0.925).Conclusion The bisexual sex exists among MSM but the consistent condoms use is at a low level.The condoms use with female among MSM are affected by multivariate factors,so correspondent interventions should be taken to avoid the risk of sexually transmitted diseases transmitted from MSM to the general population.
