阿法替尼致患者瘙痒及双手足甲沟炎1例并文献复习

摘　　要：患者女,53岁,手足反复出现红斑伴肿胀、渗出、疼痛伴背部瘙痒10个月。患者既往肺部腺癌病史,口服阿法替尼治疗5个月时出现背部瘙痒及甲沟炎症状。皮肤科情况:背部可见少许淡红斑、小丘疹及抓痕,耳廓可见淡红斑;指(趾)甲及周围软组织轻微红肿,局部皮肤温度较高,3个手指及8个足趾甲沟红肿,局部压痛,少量白色脓液由甲沟流出,双足甲缘可见少许脱屑。诊断为甲沟炎(Ⅲ度),变应性湿疹及肺腺癌。A 53-year-old female presented with repeated erythema on her hands and feet with swelling,exudation,pain and back itching for 10 months.The patient had a history of lung adenocarcinoma.She developed back pruritus and paronychia symptoms during the five months of oral afatinib treatment.The physical examination showed a few light erythema,small papules and scratches on the back,light erythema on the auricle;slight redness and swelling of the nail and surrounding soft tissue,high local skin temperature,red and swelling of three fingers and eight toenails,local tenderness;a small amount of white pus flowing out of the nail groove,and a little desquamation on the nail margin of the feet.Diagnosis:paronychia(Ⅲdegree),allergic eczema and lung adenocarcinoma.
