摘  要:目的探讨T2DM患者外周血游离mtDNA(线粒体DNA)和外周血白细胞线粒体功能对脂肪组织IR的影响。方法选取2017年1月至2019年3月于武汉大学中南医院内分泌科住院的新诊断未用药的T2DM患者80例(T2DM组),另选取同期健康人群64名为正常对照(NC)组。Seahorse XFe96检测仪检测两组白细胞线粒体功能。提取外周血mtDNA,RT-PCR检测mtDNA拷贝数,脂肪组织IR指数(Adipo-IR)评价脂肪组织IR水平,并检测血脂、BG和Ins、hsC-RP、TNF-α及活性氧簇(ROS)。结果与NC组比较,T2DM组外周血游离mtDNA、Adipo-IR升高(P<0.01)。随着Adipo-IR升高,外周血游离mtDNA水平逐渐升高,白细胞线粒体剩余呼吸功能和最大呼吸功能逐渐降低。外周血游离mtDNA与ROS、TNF-α、hsC-RP及Adipo-IR呈正相关(P<0.05或P<0.01)。外周血白细胞线粒体剩余呼吸功能和最大呼吸功能与ROS、TNF-α、hsC-RP呈负相关(P<0.05)。Logistic回归分析显示,外周血游离mtDNA,外周血白细胞线粒体剩余呼吸功能、白细胞线粒体最大呼吸功能均为脂肪组织IR的影响因素。结论外周血白细胞线粒体功能和游离mtDNA水平可能影响脂肪组织IR。Objective To investigate the effect of peripheral blood free mitochondrial DNA(mtDNA) and peripheral blood leukocyte mitochondrial function on adipose tissue insulin resistance(IR)in patients with type 2 diabetes mellitus(T2 DM).Zhongnan Hospital of Wuhan University from January 2017 to March 2019 were recruited. 64 health subjects were selected as control group. Peripheral blood leukocytes were extracted and the mitochondrial function of leukocytes was measured using a Seahorse XFe96 detector. Peripheral blood mtDNA was extracted and the copy number was detected by RT-PCR. Adipo-IR was used to evaluate adipose tissue insulin resistance.Results Compared with control group,the free mtDNA(P<0. 01)and Adipo-IR(P<0. 01)in peripheral blood of T2 DM patients were significantly increased. There was a significant positive correlation between free mtDNA and Adipo-IR in peripheral blood(r=0. 27,P<0. 01). With the increase of Adipo-IR,the level of free mtDNA in peripheral blood gradually increased;the residual respiratory function and maximum respiratory function of leukocyte mitochondria gradually decreased. Positive correlation was observed between mtDNA and ROS,TNF-α and hsC-RP(P<0. 05 or P<0. 01)in peripheral blood. Peripheral blood leukocyte mitochondrial residual respiratory function and maximum respiratory function showed significant negative correlations with ROS,TNF-α and hsC-RP(P<0. 05). Logistic multiple regression analysis showed that peripheral blood free mtDNA,peripheral blood leukocyte mitochondrial residual respiratory function and peripheral blood leukocyte mitochondrial maximum respiratory function were risk factors for Adipo-IR.Conclusion The mitochondrial function of peripheral blood leukocytes and the level of free mtDNA in peripheral blood may affect adipose tissue insulin resistance.
