摘 要:目的观察电针智三针对血管性痴呆(VaD)大鼠的治疗作用及治疗前后突触结合蛋白(Syt)-4、白细胞介素(IL)-18、过敏霉素(C3a)的表达情况。方法VaD大鼠模型采用改良Pulsinelli四血管闭塞法(4-VO)建立,随机数字表法分为模型组、电针智三针组、尼莫地平组,设假手术组进行对照。治疗组连续治疗20 d后,Morris水迷宫测试各组大鼠的运动和学习记忆。酶联免疫吸附试验测定血清中IL-18、C3a含量,Real-time PCR法检测海马CA1区Syt-4 mRNA的表达,免疫组织化学法测定海马CA1区Syt-4表达,透射电镜观察大鼠海马CA1区超微结构的改变。结果与假手术组比较,模型组平均逃避潜伏期显著延长、跨越原平台次数显著减少(P<0.05),大鼠海马CA1区Syt-4表达和血清中IL-18、C3a的含量明显升高(P<0.05);与模型组比较,电针智三针组及尼莫地平组平均逃避潜伏期显著缩短、跨越原平台次数显著增加(P<0.05),大鼠海马CA1区Syt-4表达和血清中IL-18、C3a的含量均明显降低(P<0.05)。电镜下模型组海马CA1区突触数目减少,线粒体肿胀、空泡化,细胞核膜皱缩;电针智三针组突触数量丰富、形态清晰,细胞器结构基本完整。结论电针智三针可显著改善VaD大鼠的学习记忆能力,机制可能与降低VaD大鼠海马Syt-4表达、降低血清IL-18、C3a含量,延缓海马CA1区神经元损伤有关。Objective To observe the therapeutic effect of electroacupuncture Zhisan acupuncture on vascular dementia(VD)rats and the expression of synaptophysin-4(Syt-4),interleukin-18(IL-18)and C3a before and after treatment.Methods VD rat model was established by using the modified Pulsinelli′s four vessel blocking method.They were randomly divided into VD model group,electroacupuncture Zhisan acupuncture group and nimodipine group.The sham group was set as the control group.The rats in two treatment groups were treated continuously for 20 days.After treatment,Morris water maze tests were performed to detect the rat's learning and memory ability.Blood was collected from the femoral artery,the content of IL-18 and C3a in the serum was determined by ELISA.Finally,brain hippocampal tissues were collected,the expression of Syt-4 mRNA in hippocampus CA1 area was detected by real-time PCR,the expression of Syt-4 in hippocampus CA1 area was determined by immunohistochemical method.Transmission electron microscopy(TEM)was used to observe the ultrastructure of CA1 in hippocampus of rats.Results After treatment,compared with the sham group,the average escape latency of rats in the model group was significantly prolonged,and the number of times across the primary platform was significantly reduced(P<0.05).The expression of syt-4 in the hippocampal CA1 region and the contents of il-18 and C3a in the serum of rats were significantly increased(P<0.05).Compared with the model group,the escape latency of the rats in the Electroacupuncture Zhisan acupuncture group was significantly shortened,and the number of times across the original platform was significantly increased.The difference was statistically significant(P<0.05).Syt-4 expression in the CA1 area of the hippocampus and the content of IL-18 and C3a in the serum of the rats were significantly decreased(P<0.05).Electron microscopy results showed that in the model group,the number of synapses in the hippocampus CA1 region decreased,the synaptic gaps were blurred,the mitochondria sw
