摘  要:目的探讨原发免疫性血小板减少症(ITP)患者血清细胞因子水平变化与血小板参数的相关性。方法回顾性选取2018年10月至2019年10月首都医科大学附属北京友谊医院收治的60例ITP患者为研究组,并选取同一时期的60例健康人为对照组。比较2组受试者血清中白细胞介素(IL)-33、IL-9、γ干扰素(IFN-γ)、转化生长因子-β(TGF-β)的水平;比较2组受试者血小板计数(PLT)、血小板压积(PCT)水平;采用Pearson法分析ITP患者血清细胞因子水平变化与血小板参数的相关性。结果研究组IL-33、IL-9、IFN-γ水平分别为(82.05±24.27)pg/mL、(25.93±8.55)ng/L、(284.42±44.31)pg/mL,均明显高于对照组[(35.64±12.82)pg/mL、(14.58±5.82)ng/L、(162.74±25.41)pg/mL],TGF-β水平为(1867.37±356.04)pg/mL,明显低于对照组[(2838.82±432.87)pg/mL],差异均有统计学意义(P<0.05)。研究组PLT、PCT分别为(21.53±12.34)×10^(9)/L、(0.08±0.04)%,均明显低于对照组[(153.68±32.84)×10^(9)/L、(0.25±0.08)%],差异均有统计学意义(P<0.05)。Pearson及多重性回归分析表明ITP患者IL-33、IL-9、IFN-γ水平与PLT及PCT呈负相关(P<0.05),TGF-β水平与PLT及PCT呈正相关(P<0.05)。结论ITP患者IL-33、IFN-γ、IL-9水平升高,而TGF-β水平降低,ITP患者机体内IL-33、IFN-γ、IL-9、TGF-β水平与血小板参数密切相关。Objective To investigate the correlation between serum cytokine levels and platelet parameters in patients with primary immune thrombocytopenia(ITP).Methods From October 2018 to October 2019,60 patients with ITP admitted to the Beijing Friendship Hospital,Capital Medical University were retrospectively selected as the study group,and 60 healthy people in the same period were selected as the control group.The levels of interleukin(IL)-33,IL-9,interferon-γ(IFN-γ),and transforming growth factor-β(TGF-β)in serum between the two groups were compared.The levels of platelet count(PLT)and plateletocrit(PCT)in the two groups were compared.The correlation between changes in serum cytokine levels and platelet parameters in patients with ITP was analyzed using Pearson's method.Results The levels of IL-33,IL-9,and IFN-γin the study group were(82.05±24.27)pg/mL,(25.93±8.55)ng/L,and(284.42±44.31)pg/mL,which were significantly higher than those in the control group[(35.64±12.82)pg/mL,(14.58±5.82)ng/L,(162.74±25.41)pg/mL],the level of TGF-βwas(1867.37±356.04)pg/mL,which was significantly lower than in the control group[(2838.82±432.87)pg/mL],the differences were statistically significant(P<0.05).The PLT and PCT in the study group were(21.53±12.34)×10^(9)/L and(0.08±0.04)%respectively,which were significantly lower than those in the control group[(153.68±32.84)×10^(9)/L,(0.25±0.08)%],the differences were statistically significant(P<0.05).Pearson and multiple regression analysis showed that IL-33,IL-9,IFN-γlevels were negatively correlated with PLT and PCT in ITP patients(P<0.05),TGF-βlevel was positively correlated with PLT and PCT(P<0.05).Conclusion ITP patients have higher levels of IL-33,IFN-γ,and IL-9,while TGF-βlevels are lower than normal people.The levels of IL-33,IL-9,IFN-γ,and TGF-βin ITP patients are significantly correlated with platelet parameters.
