无气腹V-NOTES与传统腹腔镜治疗卵巢囊肿临床疗效比较

摘　　要：目的比较无气腹阴道自然腔道内镜手术(vaginal natural orififice transluminal endoscopic surgery,V-NOTES)和传统腹腔镜手术在治疗卵巢囊肿患者时的疗效。方法选取2018年1月至2020年12月在沈阳市第四人民医院收治的卵巢囊肿患者52例进行回顾性分析,分为无气腹V-NOTES组和传统腹腔镜组,其中无气腹V-NOTES组21例、传统腹腔镜组31例。比较两组的手术时间、术中出血量、术后峰值体温、住院费用、术后排气时间、住院时间、术后1 d疼痛视觉模拟量表(visual analogue scale,VAS)评分情况。结果无气腹V-NOTES组的手术时间、术中出血量、术后峰值体温和住院费用与传统腹腔镜组相比,差异无统计学意义(P>0.05)。无气腹V-NOTES组的术后排气时间、住院时间短于传统腹腔镜组,术后1 d VAS评分低于传统腹腔镜组,差异有统计学意义(P<0.05)。结论无气腹V-NOTES治疗卵巢囊肿安全、可行、术后恢复快、疼痛轻,可在临床推广。Objective To compare the efficacy of gasless vaginal natural orififice transluminal endoscopic surgery and traditional laparoscopy in the treatment of ovarian cysts.Methods 52 patients with ovarian cyst admitted to the Fourth People′s Hospital of Shenyang from Jan.2018 to Dec.2020 were analyzed retrospectively.They were divided into gasless V-NOTES group and traditional laparoscopy group.There were 21 cases in gasless V-NOTES group and 31 cases in traditional laparoscopy group.The operation time,intraoperative bleeding,average peak body temperature,hospitalization expenses,postoperative exhaust time,hospitalization time,postoperative pain score of 1 day were compared between the two groups.Results There was no significant difference between gasless V-NOTES group and traditional laparoscopy group in operative time,intraoperative bleeding,average peak body temperature and hospitalization expenses(>0.05).The postoperative exhaust time and hospitalization time in gasless V-NOTES group were shorter than those in traditional laparoscopic group,and the postoperative pain score was lower than that in traditional laparoscopic group(<0.05).Conclusions Gasless V-NOTES is safe and feasible in the treatment of ovarian cyst.
