摘  要:比较白针、水针、穴位埋线3种针术刺激后海穴(GV-1)对便秘大鼠的作用及其对肠神经递质及5-羟色胺(5-HT)信号途径的影响。选用SD大鼠60只,随机分为A~F组(对照组、模型组、西药组、白针组、水针组、埋线组),共6组,每组10只。除空白组外,其余各组大鼠每天灌胃盐酸洛哌丁胺,建立便秘模型,盐酸洛哌丁胺一直用至试验结束以维持便秘。便秘6 d,C组应用1.3 mg/(kg·d)莫沙必利灌胃,D组针刺GV-120 min,E组GV-1注射0.1 mL VB12,F组GV-1埋入羊肠线。除F组仅处理1次外,其余各组每天1次,连续7 d。结果显示,3种针术均显著提高便秘大鼠粪便数量(P<0.05),加速小肠推进率(P<0.01),其中埋线组的小肠推进率及水针组的粪便粒数效果最佳,与西药组无显著性差异。ELISA结果显示,3种针术均显著提高便秘大鼠肠道P物质(SP)水平(P<0.01),其中以水针及埋线作用最佳;同时3种针术显著降低便秘大鼠结肠中血管活性肠肽(VIP)、5-HT含量。Q-PCR结果显示,GV-13种针术均显著提高5-HT4受体(5-HT4R)、5-HT3受体(5-HT3R)和5-HT重摄取转运体(SERT)的mRNA转录水平,埋线组达到或优于西药治疗组。结果表明,针刺GV-1可以缓解大鼠便秘,改善结肠组织结构,调节SP、VIP、5-HT含量及5-HT信号途径。3种针术均达到西药治疗组效果,其中埋线与水针效果优于白针。The present study compared the effect of using three kinds of acupuncture therapies to stimulate Houhai(GV-1)acupoint on constipation rats and explored the regulating effect on intestinal neurotransmitter and 5-hydroxytryptamine(5-HT)signaling pathways.Sixty SD rats were randomly divided into 6groups(n=10each),including blank control(group A),constipation model(group B),western medicine(group C),dry needle(group D),GV-1injection(group E),GV-1embedding(group F).Except the group A,the rats in other groups were given loperamide daily to establish and maintain the constipation.On the 6th day of constipation,group C were gavaged with 1.3mg/(kg·d)mosapride,group D were stimulated with dry needling for 20min at GV-1acupoint,group E were injected with 0.1mL VB12at GV-1acupoint,group F received catgut embedding at GV-1acupoint.The results showed that all three acupuncture therapies could significantly increase the number of feces and improve the intestinal propulsion rate,and significantly increase the level of substance P(SP)in colon tissues(P<0.01),while the contents of vasoactive intestinal peptide(VIP)and 5-HT were significantly reduced by the acupuncture treatments,and the therapy of catgut embedding at GV-1acupoint reached the same therapeutic effects as western medicine(the group C).The transcriptional levels of 5-HT4receptor(5-HT4R),5-HT3receptor(5-HT3R)and serotonin reuptake transporter(SERT)were significantly increased in all acupuncture groups,and the GV-1acupoint embedding group reached or outperformed the western drug treatment group.In conclusion,the three acupuncture therapies at GV-1acupoint can relieve rat constipation,improve colon histological structure,regulate the contents of SP,VIP,5-HT and 5-HT pathways,all three acupuncture therapies can achieve the same effect of western medicine,the GV-1acupoint embedding and acupoint injection therapies are better than dry needling therapy.
