摘　　要：目的:探讨他达拉非联合西地那非治疗勃起功能障碍(ED)的临床效果和对阴茎血流动力学的影响。方法:选取2018年8月—2020年8月治疗的ED患者84例,按随机数字表法分为对照组和观察组,每组42例。对照组口服西地那非治疗,观察组在此基础上口服他达拉非治疗。比较两组临床疗效、勃起功能、阴茎血流动力学和不良反应情况。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05);观察组治疗后国际勃起功能评分量表(IIEF-5)、勃起硬度(EHS)和勃起质量量表(QEQ)评分高于对照组,差异有统计学意义(P<0.05);观察组治疗后收缩期峰值流速(PSV)低于对照组,舒张末期流速(EDV)、阻力指数(RI)高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率相比,差异无统计学意义(P>0.05)。结论:他达拉非联合西地那非可增强ED患者勃起功能,改善阴茎血流动力学,提高阴茎坚硬度,且安全性高,利于改善患者性生活质量。Objective:To investigate the clinical effect of tadalafil combined with sildenafil in the treatment of erectile dysfunction(ED)and its influence on penile hemodynamics.Methods:A total of 84 ED patients treated in hospital from August 2018 to August 2020 were selected and divided into two groups according to the random number table method,with 42 cases in each group.The control group was treated with sildenafil orally as needed,and the observation group was treated with tadalafil daily.The clinical efficacy,erectile function,penile hemodynamics and adverse reactions were compared between the two groups.Results:The total effective rate of treatment in the observation group was higher than the control group.There was a statistical difference(P<0.05).The international erectile function rating scale(IIEF-5),erectile hardness(EHS)and erectile quality scale(QEQ)scores of the observation group after treatment were higher than the control group,there was significient difference(P<0.05).The peak systolic velocity(PSV)of the observation group after treatment was lower than that of the control group,and the end-diastolic velocity(EDV)and resistance index(RI)were higher than those of the control group,the difference was statistically significant(P<0.05).The difference between the two groups of adverse reactions was not statistically significant(P>0.05).Conclusion:Tadalafil combined with sildenafil can enhance erectile function,improve penile hemodynamics,and increase penile stiffness in patients with ED.It is safe and beneficial to improve the sexual life of patients.
