摘  要:目的分析血清微小RNA-29a水平与妊娠糖尿病(GDM)孕妇母婴结局的相关性。方法纳入245例GDM孕妇,233例随访至分娩结束,根据母婴结局将其分为对照组(n=168)和不良结局组(n=65)。比较两组孕妇的一般资料、血清微小RNA-29a水平和其他实验室指标;应用多因素Logistic回归模型分析GDM孕妇发生母婴不良结局的影响因素;应用受试者工作特征曲线分析危险因素预测GDM孕妇发生母婴不良结局的价值。结果完成随访的233例GDM孕妇中,65例GDM孕妇出现母婴不良结局,发生率为27.90%。不良结局组的孕期增重大于对照组,空腹血糖、糖化血红蛋白(HbA1c)和微小RNA-29a水平均高于对照组(均P<0.05)。多因素Logistic回归分析显示,孕期增重、空腹血糖、HbA1c和微小RNA-29a水平均是GDM孕妇发生母婴不良结局的独立危险因素(均P<0.05)。微小RNA-29a预测GDM孕妇发生母婴不良结局的曲线下面积为0.821,大于孕期增重、空腹血糖、HbA1c的曲线下面积(均P<0.05)。结论微小RNA-29a水平升高的GDM孕妇发生母婴不良结局的风险增高,该指标有助于GDM孕妇母婴结局的评估。Objective To analyze the correlation of serum microRNA-29a level with maternal and neonatal outcomes in pregnant women with gestational diabetes mellitus(GDM).Methods A total of 245 pregnant GDM women were enrolled,233 cases were followed up until the end of delivery and then allocated to control group(n=168)or poor outcome group(n=65)according to the maternal and neonatal outcomes.The general data,serum microRNA-29a level and other laboratory indexes of pregnant women were compared between the two groups.Multivariate Logistic regression model was used to analyze the factors affecting the incidence of maternal and neonatal adverse outcomes in pregnant GDM women.Receiver operating characteristic curve was employed to analyze the predictive value of risk factors for maternal and neonatal adverse outcomes in pregnant GDM women.Results Among the 233 pregnant women who completed follow-up,65 pregnant women with GDM suffered from maternal and neonatal adverse outcomes,and the incidence rate was 27.90%.By comparison with the control group,the poor outcome group had increased gestational weight gain,higher FBG,hemoglobin A1c(HbA1c)and microRNA-29a levels(all P<0.05).Multivariate Logistic regression analysis revealed that gestational weight gain,FBG,HbA1c and microRNA-29a levels were all independent risk factors for maternal and neonatal adverse outcomes in pregnant GDM women(all P<0.05).For predicting the incidence of maternal and neonatal adverse outcomes in pregnant GDM women,microRNA-29a achieved an area under the curve of 0.821 larger than that of gestational weight gain,FBG or HbA1c(all P<0.05).Conclusion Pregnant GDM women with elevated microRNA-29a level have a higher risk of maternal and neonatal adverse outcomes,and that indicator is conductive to evaluating the maternal and neonatal outcomes of pregnant GDM women.
