杜仲颗粒对高血压肾病患者血管内皮功能、血压及尿微量白蛋白水平的影响

摘　　要：目的探讨杜仲颗粒对高血压肾病患者血管内皮功能、血压及尿微量白蛋白水平的影响。方法将128例高血压肾病患者随机分为观察组和对照组,各64例。两组患者均给予一般治疗及口服苯磺酸氨氯地平片治疗,在此基础上观察组给予口服杜仲颗粒。治疗前和治疗3个月后,比较两组患者的血压、心率、24 h尿微量白蛋白以及血清一氧化氮、一氧化氮合酶(NOS)、内皮素-1、超氧化物歧化酶(SOD)水平。结果治疗后,两组患者的收缩压、舒张压及心率均低于治疗前(均P<0.05),但两组上述各指标比较差异均无统计学意义(均P>0.05);两组患者的24 h尿微量白蛋白和血清内皮素-1水平均较治疗前降低,血清一氧化氮、NOS、SOD水平均较治疗前升高,且观察组以上指标改善均优于对照组(均P<0.05)。结论在常规治疗的基础上加用杜仲颗粒可有效地降低高血压肾病患者的血压,且较之于常规治疗能更好地改善高血压肾病患者的血管内皮功能,降低尿微量白蛋白水平,从而更好地发挥缓解病变血管的收缩和保护肾脏功能的作用。Objective To explore the effects of Duzhong Granules on vascular endothelial function,blood pressure and microalbuminuria level in patients with hypertensive nephropathy.Methods Totally 128 patients with hypertensive nephropathy were randomized to observation group(n=64)or control group(n=64).Both groups received general treatment and oral administration of Amlodipine Besylate Tablets,based on which the observation group received Duzhong Granules orally.Before treatment and after three months of treatment,multiple indicators were compared between the two groups,including blood pressure,heart rate,24-hour microalbuminuria,serum nitric oxide,nitric oxide synthase(NOS),endothelin-1 and superoxide dismutase(SOD)levels.Results After treatment,systolic and diastolic blood pressure and heart rate were lower in both groups as compared with those before treatment(all P<0.05),but no statistically significant difference was found in the aforementioned indicators between the two groups(all P>0.05);the levels of 24-hour microalbuminuria and serum endothelin-1 were decreased,and serum nitric oxide,NOS and SOD levels were increased in both groups as compared with the levels before treatment,moreover,the observation group was superior to the control group in the improvements of the aforementioned indicators(all P<0.05).Conclusion Additional use of Duzhong Granules on the basis of conventional therapy can effectively reduce blood pressure of patients with hypertensive nephropathy,moreover,compared with conventional therapy,it can better improve vascular endothelial function and decrease microalbuminuria level in patients with hypertensive nephropathy,thus exerting preferable effects on diminishing contraction of affected vessels and protecting renal function.
