摘 要:目的分析黄芩苷对泛发性脓疱型银屑病(GPP)患者机体Th1/Th2失衡、T盒子转录因子(T-bet)/GATA结合蛋白3(GAGA-3)表达的影响。方法(1)获取36例GPP患者的外周血标本以分离外周血单个核细胞(PBMC),用含0μg/mL、50μg/mL、100μg/mL、200μg/mL、300μg/mL、400μg/mL黄芩苷干预48 h后,检测PBMC活力以筛选适宜的药物干预浓度。(2)随机选取6例GPP患者的PBMC,加入适宜浓度的黄芩苷溶液处理细胞,以0μg/mL黄芩苷干预的细胞作为阴性对照组。干预48 h后,检测Th1和Th2比例,Th1相关细胞因子[白细胞介素(IL)-2、γ-干扰素、肿瘤坏死因子β(TNF-β)]和Th2相关细胞因子(IL-4、IL-5、IL-6、IL-10、IL-13)表达水平,以及T-bet mRNA和GATA-3 mRNA的表达水平。结果(1)与其他浓度黄芩苷相比,300μg/mL、400μg/mL黄芩苷干预后的PBMC存活率降低(P<0.05),故选取200μg/mL作为最适宜干预浓度。(2)与阴性对照组比较,200μg/mL黄芩苷组的Th1比例以及IL-2、γ-干扰素、TNF-β、T-bet mRNA表达水平降低,而Th2比例以及IL-4、IL-5、IL-6、IL-10、IL-13、GATA-3 mRNA表达水平升高(均P<0.05)。结论黄芩苷能抑制Th1转录因子T-bet的表达,下调Th1细胞水平,并增强Th2转录因子GATA-3的表达,上调Th2细胞水平,从而促进Th1细胞向Th2漂移。Objective To analyze the effect of baicalin on Th1/Th2 imbalance and T-box transcription factor(T-bet)/GATA binding protein 3(GAGA-3)expression in patients with generalized pustular psoriasis(GPP).Methods(1)Peripheral blood samples were harvested from 36 GPP patients to isolate peripheral blood mononuclear cells(PBMC),then the cells were treated with 0μg/mL,50μg/mL,100μg/mL,200μg/mL,300μg/mL and 400μg/mL baicalin for 48 hours to detect the viability of PBMC so as to determine the appropriate drug concentrations for intervention.(2)PBMC collected from six GPP patients randomly selected were added with appropriate concentrations of baicalin solution for cell treatment,and PBMC treated with 0μg/mL baicalin acted as negative control group.After 48 hours of intervention,the proportions of Th1 and Th2,the expression levels of Th1-related cytokines(interleukin[IL]-2,interferon-γ,tumor necrosis factorβ[TNF-β])and Th2-related cytokines(IL-4,IL-5,IL-6,IL-10,IL-13),and the expression levels of T-bet mRNA and GATA-3 mRNA were determined.Results(1)Compared with other concentrations of baicalin,intervention with 300μg/mL or 400μg/mL baicalin produced a decline in survival rate of PBMC(P<0.05),thereby 200μg/mL served as the optimum intervention concentration.(2)Compared with the negative control group,the 200μg/mL baicalin group exhibited lower proportion of Th1,reduced expression levels of IL-2,interferon-γ,TNF-βand T-bet mRNA,higher proportion of Th2,and elevated expression levels of IL-4,IL-5,IL-6,IL-10,IL-13 and GATA-3 mRNA(all P<0.05).Conclusion Baicalin can inhibit the expression of Th1 transcription factor T-bet,down-regulate the level of Th1 cells,enhance the expression of Th2 transcription factor GATA-3,and up-regulate the level of Th2 cells,thus promoting the shift of Th1 cells to Th2 cells.
