摘  要:目的:比较基于扫描范围中心层面有效直径ED和水当量直径WD的体型特异性剂量评估SSDE_(ED)与SSDE_(WD)在成人头颅CT扫描中辐射剂量的差异。方法:连续搜集东芝公司64排Aquilion CT机上的108例成人患者头部CT螺旋扫描数据,男52例,女56例,记录每个患者的容积CT剂量指数CTDIvol,分别计算ED,WD,相应的转换系数f_(ED),f_(WD)和体型特异性剂量指数SSDE_(ED),SSDE_(WD)。比较两种方法对应的体径,转换系数和体型特异性剂量的差异并比较男女间ED、WD、f_(ED)、f_(WD)、SSDE_(ED)、SSDE_(WD)的差异。结果:头部扫描两种体型特异性剂量算法的平均体径WD比ED增加7.59%,平均转换系数f_(WD)比f_(ED)降低4.89%,平均体型特异性剂量评估SSDE_(WD)比SSDE_(ED)降低4.89%,差异均有统计学意义(t=59.83,-61.28,-61.27,P<0.05)。SSDE_(ED)和SSDE_(WD)比CTDIvol分别低3.22%和7.95%。男女ED、WD男组高于女组;f_(ED)、f_(WD)、SSDE_(ED)、SSDE_(WD)男组则均低于女组,差异也都有统计学意义(t=10.89,9.44,-10.93,-9.48,-10.93,-9.48,P<0.05)。结论:SSDE_(ED)和CTDIvol都高估了成人头部CT扫描的辐射剂量,SSDE_(WD)能更加准确的评估成人头颅CT扫描的辐射剂量。Objective:To compare the difference between the size specific dose estimate effective diameter(SSDE ED)and the size specific dose estimate water diameter(SSDE WD)for evaluating radiation dose based in central slice effective diameter(ED)and water equivalent diameter(WD)in adult skull CT scanning.Methods:The CT spiral scan data of 108 adult patients(male 52,female 56)on 64-row CT scanner(Aquilion,Toshiba Company)were collected continuously.CTDIvol value of each patient in the dose report was recorded.The ED,WD,the conversion coefficient of_(ED)(f_(ED)),the conversion coefficient of_(WD)(f_(WD)),SSDE ED and SSDE WD were calculated.The differences of body diameter,conversion coefficient and SSDE which were calculated with two methods were compared.The differences of_(ED),WD,f_(ED),f_(WD),SSDE ED and SSDE WD in male group and female group were compared respectively.Results:The WD is 7.59%more than ED(t=59.83,P<0.05).The f_(WD) is 4.89%less than f_(ED)(t=-61.58,P<0.05).The SSDE WD is 4.89%less than SSDE ED(t=-61.27,P<0.05).SSDE ED is 3.22%lower than CTDIvol.SSDE WD is 7.95%lower than CTDIvol.The ED and WD in male group are higher than which in female group respectively(t=10.89,9.44,P<0.05).The f_(ED),f_(WD),SSDE ED and SSDE WD in male groupare are lower than which in female group respectively(t=-10.93,-9.48,-10.93,-9.48,P<0.05).Conclusion:Both SSDE ED and CTDIvol may be overestimated the radiation dose of adult head CT scanning.SSDE WD may be more accurate for evaluation the radiation dose of adult head CT scanning.
