温针灸治疗原发性痛经取穴规律的数据挖掘研究

摘　　要：目的:探讨温针灸治疗原发性痛经的取穴规律。方法:检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库有关温针灸治疗原发性痛经的文献,提取腧穴处方,建立数据库,对腧穴归经、体表分布、特定穴进行描述性分析,运用SPSS20软件对高频腧穴进行聚类分析和关联规则分析。结果:最终纳入63篇文献,涉及41个腧穴,使用频次349次,三阴交应用频率最高,关元、气海、次髎、足三里、地机等为高频腧穴;常用经脉是脾经和任脉;腧穴主要分布在下肢和腹部;特定穴以募穴和交会穴最常用;高频腧穴聚类分析得出7个有效聚类群,关联性最强的穴位组合为关元-三阴交。结论:温针灸疗法原发性痛经选用腧穴多具有温补性;重视疼痛部位腧穴的应用,善于特定穴的应用,尤其是募穴和交会穴,核心腧穴组合为关元-三阴交,以远近配穴为主。Objective:To explore the rules of acupoint selection in the treatment of primary dysmenorrhea with warm acupuncture.Methods:We searched CNKI,Wanfang database,VIP database and Chinese biomedical literature database for literatures on warm acupuncture treatment of primary dysmenorrhea,extracted acupoint prescriptions,established database,and made descriptive analysis on acupoint meridian tropism,body surface distribution and specific acupoints.SPSS 20 software was used for cluster analysis and association rule analysis on high-frequency acupoints.Results:63 literatures were finally included,and 41 acupoints were involved,and the frequency of application was 349 times.Sanyinjiao acupoints were the most frequently used acupoints,Guanyuan,Qihai,Cimiu,Zusanli,and Diji were high frequency acupoints;the common meridians are spleen meridians and Ren meridians;acupoints were mainly distributed in lower limbs and abdomen;the most common specific acupoints are Mu acupoint and intersection acupoint;the cluster analysis of high frequency acupoints showed 7 effective cluster groups,and the most relevant acupoint combination was Guanyuan-Sanyinjiao.Conclusion:Acupoints used in warm acupuncture therapy for primary dysmenorrhea are mostly warming and tonifying;pay attention to the application of acupoints in pain areas,and be good at the application of specific acupoints,especially the Mu acupoint and the intersection acupoint.The core acupoint combination is Guanyuan-Sanyinjiao,with the far and near acupoints as the main combination.
