温盐水联合喜辽妥预防极低出生体重早产儿PICC置管后机械性静脉炎的作用

摘　　要：目的 探讨温盐水联合多磺酸粘多糖乳膏(商品名:喜辽妥)在预防极低出生体重早产儿经外周静脉置入中心静脉导管(PICC)置管后机械性静脉炎中的作用。方法 100例PICC置管的极低出生体重早产儿,随机分为对照组和试验组,各50例。对照组采用常规的PICC置管方法 ,试验组在对照组的基础上采用温盐水泡管、送管,置管后涂抹喜辽妥的方法。比较两组机械性静脉炎发生率、机械性静脉炎发生程度。结果 试验组机械性静脉炎的发生率10.0%低于对照组的36.0%,差异有统计学意义(P<0.05)。试验组机械性静脉炎的发生程度低于对照组,差异有统计学意义(P<0.05)。结论 温盐水联合喜辽妥可以预防极低出生体重早产儿PICC置管后机械性静脉炎的发生,降低机械性静脉炎的发生率,减轻机械性静脉炎的发生程度。Objective To discuss the effect of warm saline combined with mucopolysaccharidepolysulfate cream (trade name: Hirudoid) in prevention of mechanical phlebitis after peripherally inserted centralcatheter (PICC) placement in very-low-birth-weight preterm infants. Methods A total of 100 cases of very-lowbirth-weight premature infants with PICC placement were randomly divided into control group and experimentalgroup, with 50 cases in each group. The control group was treated with conventional PICC catheterization, whilethe experimental group was treated with warm saline bubbling tube, feeding tube and applying Hirudoid aftercatheterization. The incidence of mechanical phlebitis and degree of mechanical phlebitis were compared betweenthe two groups. Results The overall incidence of mechanical phlebitis 10.0% in the experimental group was lowerthan 36.0% in the control group, and the difference was statistically significant (P<0.05). The degree of mechanicalphlebitis in the experimental group was lower than that in the control group, and the difference was statisticallysignificant (P<0.05). Conclusion Combination of warm saline and Hirudoid can prevent the occurrence ofmechanical phlebitis after PICC placement in very-low-birth-weight premature infants, reduce the incidence ofmechanical phlebitis and reduce the degree of mechanical phlebitis.
