Ⅰ期超声乳化联合张力环植入Ⅱ期囊袋固定人工晶体治疗大范围晶体脱位临床疗效观察

摘　　要：目的 观察Ⅰ期超声乳化联合张力环植入Ⅱ期囊袋固定人工晶体治疗大范围晶体脱位临床疗效。方法 21例(21眼)外伤性白内障合并晶体半脱位(脱位范围5~7点)患者,均接受Ⅰ期超声乳化联合张力环植入Ⅱ期囊袋固定人工晶体治疗,观察治疗结果、不同时间段的视力、术后并发症发生情况。结果 21眼均顺利将晶体囊袋张力环(CTR)植入,无后囊膜破裂。其中,植入晶体囊袋内占57.14%(12/21),植入晶体囊袋前占42.86%(9/21)。术后1 d, 5眼眼压增高。随访观察1年,仅有1眼眼压超过正常范围。视力>0.6占28.57%(6/21),视力0.4~0.6占66.67%(14/21),视力≤0.3占4.76%(1/21)。所有患者散瞳,超声生物显微镜(UBM)检查CTR均位于囊袋内,人工晶体在位无晃动,且晶体光学区位于视轴处,无偏心和倾斜。4眼出现后囊膜混浊,占19.05%(4/21)。21眼均未出现视网膜脱离、黄斑囊样水肿和眼内炎。结论 采用Ⅰ期超声乳化联合张力环植入Ⅱ期囊袋固定人工晶体治疗大范围晶体脱位患者,能获得较为显著的治疗效果,不仅有助于恢复患者的视力,稳定眼压,且安全系数高,值得应用。Objective To observe the clinical efficacy of stageⅠphacoemulsification combined with tension ring implantation of stageⅡcapsular bag fixed intraocular lens in the treatment of large-scale lens dislocation.Methods There were 21 patients(21 involved eyes)with traumatic cataract and subluxation of lens(dislocation range 5-7 clock hours),and all patients were treated with stageⅠphacoemulsification combined with tension ring implantation of stageⅡcapsular bag fixed intraocular lens.The treatment results,visual acuity at different time periods,and postoperative complications were observed.Results The capsular tension ring(CTR)was successfully implanted into the capsular bag in all 21 eyes,with no posterior capsular rupture.Of these,57.14%(12/21)were implanted in the lens capsular bag and 42.86%(9/21)were implanted before the lens capsular bag.At 1 d after surgery,the intraocular pressure was increased in 5 eyes.At 1 year follow-up,only 1 eye had an intraocular pressure exceeded the normal range.28.57%(6/21)had visual acuity>0.6,66.67%(14/21)had visual acuity 0.4-0.6,and 4.76%(1/21)had visual acuity≤0.3.All patients were dilated,and the CTR was located in the capsular bag by ultrasound biomicroscopy(UBM),and the intraocular pressure was in place without shaking,and the lens optic zone was located at the visual axis,without eccentricity or tilt.Posterior capsule opacity occurred in 4 eyes,accounting for 19.05%(4/21).No retinal detachment,cystoid macular edema and endophthalmitis occurred in 21 eyes.Conclusion Combination of stageⅠphacoemulsification and tension ring implantation of stageⅡcapsular bag fixed intraocular lens has significant effect on patients with large-scale lens dislocation,which is not only helpful to restore the vision of patients,but also stable intraocular pressure,with high safety factor,which is worthy of application.
