摘 要:目的 分析不同浓度罗哌卡因用于超声引导下肌间沟臂丛神经阻滞的麻醉效果。方法 52例接受超声引导下肌间沟臂丛神经阻滞手术的患者,按完全随机法分为对照组和观察组,各26例。对照组给予0.3%罗哌卡因麻醉,观察组给予0.5%罗哌卡因麻醉。比较两组患者麻醉起效时间、不良反应发生情况、麻醉效果、麻醉指标。结果 观察组患者的正中神经、尺神经、桡神经和肌皮神经的麻醉起效时间分别为(8.20±1.08)、(6.99±1.32)、(8.25±1.19)、(6.44±1.62)min,均短于对照组的(9.68±1.13)、(9.15±1.14)、(10.96±1.42)、(7.97±1.34)min,差异均具有统计学意义(P<0.05)。观察组不良反应发生率7.69%低于对照组的30.77%,麻醉有效率96.15%高于对照组的73.08%,镇痛持续时间(8.36±0.54)h长于对照组的(7.27±0.28)h,运动阻滞恢复时间(7.65±0.68)h短于对照组的(8.97±0.76)h,差异均具有统计学意义(P<0.05)。结论 对于超声引导下肌间沟臂丛神经阻滞手术,给予不同浓度的罗哌卡因均可以起到一定的麻醉效果,但给予0.5%罗哌卡因进行麻醉,有利于缩短起效时间,减少麻醉引起的不良反应,麻醉效果更好,其临床应用价值较高,值得推广。Objective To analyze the anesthesia effects of different concentrations of ropivacaine for ultrasound-guided intermuscular groove brachial plexus block.Methods A total of 52 patients with ultrasoundguided intermuscular groove brachial plexus block were randomly divided into control group and observation group,with 26 cases in each group.The control group received 0.3%ropivacaine for anesthesia,while the observation group received 0.5%ropivacaine for anesthesia.Patients in both groups were compared in terms of onset time of anesthesia,occurrence of adverse reactions,anesthetic effect,and anesthesia indicators.Results The onset time of median nerve,ulnar nerve,radial nerve and musculocutaneous nerve of the observation group were(8.20±1.08),(6.99±1.32),(8.25±1.19),(6.44±1.62)min,which were shorter than(9.68±1.13),(9.15±1.14),(10.96±1.42)and(7.97±1.34)min of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions 7.69%in the observation group was significantly lower than 30.77%in the control group,and the effective rate 96.15%of anesthesia in the observation group was higher than 73.08%in the control group,and the duration of analgesia(8.36±0.54)h was longer than(7.27±0.28)h in the control group,and the recovery time of motor block(7.65±0.68)h in the observation group were shorter than(8.36±0.54)and(8.97±0.76)h in the control group.All the difference was statistically significant(P<0.05).Conclusion For ultrasound-guided intermuscular groove brachial plexus block,the administration of different concentrations of ropivacaine plays a certain role in anesthesia,but the administration of 0.5%ropivacaine for anesthesia is beneficial to shorten the onset time,reduce the adverse reactions caused by anesthesia.It has high clinical application value,and is worthy of promotion.
