关节镜辅助下踝关节融合术治疗足踝创伤性关节炎的效果观察

摘　　要：目的 分析关节镜辅助下踝关节融合术治疗足踝创伤性关节炎的临床效果。方法 60例足踝创伤性关节炎患者,采用计算机随机分配法分为对照组和实验组,每组30例。对照组行踝关节融合术治疗,实验组行关节镜辅助下踝关节融合术治疗。对比两组手术指标、愈合时间及关节功能评分、视觉模拟评分法(VAS)评分、日常生活能力量表(ADL)评分。结果 实验组手术时间(71.14±6.25)min短于对照组的(76.14±8.25)min,术中出血量(50.45±5.58)ml少于对照组的(65.25±7.39)ml,手术切口(4.55±0.13)cm短于对照组的(7.20±0.35)cm,差异均有统计学意义(P<0.05)。实验组愈合时间(9.58±2.14)d短于对照组的(14.20±3.11)d,关节功能评分(85.47±8.62)分高于对照组的(76.52±5.12)分,差异均有统计学意义(P<0.05)。治疗后,实验组VAS评分(2.24±0.14)分低于对照组的(4.98±1.14)分,差异有统计学意义(P<0.05)。治疗后,实验组ADL评分(91.52±7.42)分高于对照组的(77.96±7.41)分,差异有统计学意义(P<0.05)。结论 足踝创伤性关节炎患者应用关节镜辅助下踝关节融合术治疗,其效果显著,能够有效改减少术中出血量,缩短住院时间,促进患者康复,值得临床推广和运用。Objective To analyze the clinical effect of arthroscopic ankle arthrodesis in the treatment of traumatic arthritis of foot and ankle.Methods A total of 60 patients with traumatic arthritis of foot and ankle were divided into control group and experimental group according to computer random grouping method,with 30 cases in each group.The control group was treated with ankle arthrodesis,and the experimental group was treated with arthroscopic ankle arthrodesis.Both groups were compared in terms of surgical indicators,healing time,joint function score,visual analogue scale(VAS)score,and activities of daily living(ADL)score.Results The operation time(71.14±6.25)min of the experimental group was shorter than(76.14±8.25)min of the control group,the intraoperative blood loss(50.45±5.58)ml was less than(65.25±7.39)ml of the control group,and the surgical incision was(4.55±0.13)cm was shorter than(7.20±0.35)cm of the control group.All the differences were statistically significant(P<0.05).The healing time(9.58±2.14)d of the experimental group was shorter than(14.20±3.11)d of the control group,and the joint function score(85.47±8.62)points was higher than(76.52±5.12)points of the control group.All the differences were statistically significant(P<0.05).After treatment,VAS score(2.24±0.14)points of the experimental group was lower than(4.98±1.14)points of the control group,and the difference was statistically significant(P<0.05).After treatment,ADL score(91.52±7.42)points of the experimental group was higher than(77.96±7.41)points of the control group,and the difference was statistically significant(P<0.05).Conclusion Arthroscopic ankle arthrodesis has significant effect on patients with traumatic arthritis of foot and ankle,and it can effectively reduce the intraoperative blood loss,shorten the hospitalization time,and promote the recovery of patients,which is worthy of clinical promotion and application.
