摘  要:目的探讨血清血红素氧合酶-1(HO-1)、血管生成素-2(Ang-2)、多配体蛋白聚糖-4(Syndecan-4)在判断急性肺炎患儿病情严重程度及预后的意义。方法选取2018年1月至2020年1月在本院接受治疗的急性肺炎患儿129例作为观察组,根据患儿病情轻重分为重症肺炎组(n=50)、轻症肺炎组(n=79);根据预后情况分为预后良好组(n=105)和预后不良组(n=24)。另选取本院健康体检儿童80例作为对照组。采用酶联免疫吸附法检测各组受试者血清HO-1、Ang-2、Syndecan-4水平。采用Spearman相关系数分析血清HO-1、Ang-2、Syndecan-4与急性生理和慢性健康状况(APACHEⅡ)评分和序贯性器官功能衰竭评估(SOFA)评分相关性。采用受试者工作特征(ROC)曲线分析血清HO-1、Ang-2、Syndecan-4对急性肺炎患儿预后的诊断价值。结果观察组血清HO-1、Ang-2、Syndecan-4水平与对照组相比,差异有统计学意义(P<0.05)。重症肺炎组血清HO-1、Ang-2、Syndecan-4、APACHEⅡ评分和SOFA评分与轻症肺炎组相比,差异有统计学意义(P<0.05)。预后不良组血清HO-1、Ang-2、Syndecan-4水平与预后良好组相比,差异有统计学意义(P<0.05)。Spearman相关性分析结果显示:血清HO-1、Ang-2分别与APACHEⅡ评分和SOFA评分呈正相关关系,血清Syndecan-4与APACHEⅡ评分和SOFA评分呈负相关关系(P<0.05)。ROC曲线显示:血清HO-1、Ang-2、Syndecan-4及3项联合检测预测急性肺炎患儿不良预后的AUC分别为0.879、0.822、0.861和0.931。结论急性肺炎患儿血清HO-1、Ang-2升高和Syndecan-4降低与急性肺炎患儿病情严重程度及预后不良有关;联合检测血清HO-1、Ang-2、Syndecan-4可用于预测急性肺炎患儿的临床预。Objective To explore the significance of serum heme oxygenase-1(HO-1),angiopoietin-2(Ang-2),and syndecan-4(Syndecan-4)in evaluating the conditions and prognosis of children with acute pneumonia.Methods A total of 129 children with acute pneumonia who were treated in the hospital from January 2018 to January 2020 were selected as the observation group.These children were divided into severe pneumonia group(n=50)and mild pneumonia group(n=79)according to their severity;According to the prognosis,they were divided into good prognosis group(n=105)and poor prognosis group(n=24).Besides,80 healthy children in the hospital were selected as the control group.Enzyme-linked immunosorbent assay was used to detect the serum levels of HO-1,Ang-2,and Syndecan-4 in each group.Spearman correlation was used to analyze the correlation between serum HO-1,Ang-2,Syndecan-4,and acute physiology and chronic health evaluationⅡ(APACHEⅡ)score,and sequential organ failure assessment(SOFA)score.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficacy of serum HO-1,Ang-2,and Syndecan-4 on the prognosis of children with acute pneumonia.Results The serum levels of HO-1,Ang-2,and Syndecan-4 in the observation group were significantly different from those in the control group,and the differences were statistically significant(P<0.05).The serum HO-1,Ang-2,Syndecan-4,APACHEⅡscores and SOFA scores in the severe pneumonia group were significantly different from those in the mild pneumonia group,and the differences were statistically significant(P<0.05).The serum levels of HO-1,Ang-2,and Syndecan-4 in the poor prognosis group were significantly different from those in the good prognosis group,and the differences were statistically significant(P<0.05).Spearman correlation analysis showed that serum HO-1 and Ang-2 were positively correlated with APACHEⅡscore and SOFA score,respectively,and serum Syndecan-4 was negatively correlated with APACHEⅡscore and SOFA score(P<0.05).The ROC curve showed that the AUC of
