摘  要:目的:探讨低剂量CT检查对新生儿气漏综合征的诊断应用,提高对气漏及导致气漏的原发疾病的诊断能力。方法:回顾性分析2015-01~2019-12间本院确诊的37例气漏患儿的低剂量CT表现,所有病例均使用联影UCT760扫描仪接受胸部或腹部低剂量扫描,管电流低于25mAs,所有图像经过薄层0.625~1.0mm重建后分析诊断。结果:37例患儿中1例为胃壁穿孔导致气腹,其余36例均为胸部气漏,发现单纯单侧气胸4例,单侧气胸合并间质内气肿1例,单侧气胸合并对侧肺间质内气肿4例,单侧气胸合并纵隔气肿1例,单侧气胸合并间质内气肿并纵隔气肿1例。双侧气胸4例,双侧气胸合并纵隔气肿1例,双侧气胸合并间质内气肿14例,双侧气胸合并纵隔气肿合并间质内气肿6例。32例患儿肺内存在原发疾病:吸入综合征、新生儿肺炎、新生儿呼吸窘迫综合征及湿肺。结论:低剂量CT对新生儿复杂的气漏综合征诊断率为100%,同时对原发肺内疾病的诊断更具价值。Objective:To explore the application of low-dose CT in the diagnosis of neonatal air leak syndrome,and to improve the diagnostic ability of air leak and the primary diseases leading to air leak.Methods:the low dose CT findings of38 children with air leakage diagnosed in our hospital from January 2015 to December 2019 were retrospectively analyzed.All patients were scanned with uct760 scanner,and the tube current was less than 25 mas.All the images were reconstructed by0.625 mm~1.0 mm thin section and analyzed.Results:One of the 37 cases was pneumoperitoneum caused by perforation of gastric wall,and the other 36 cases were chest air leakage.,and it was found that there were 4 cases of simple unilateral pneumothorax,1 case of unilateral pneumothorax with interstitial emphysema,4 cases of unilateral pneumothorax with contralateral pulmonary interstitial emphysema,1 case of unilateral pneumothorax with mediastinal emphysema,and 1 case of unilateral pneumothorax with interstitial emphysema and mediastinal emphysema.There were 4 cases of bilateral pneumothorax,1 case of bilateral pneumothorax with mediastinal emphysema,14 cases of bilateral pneumothorax with interstitial emphysema,and6 cases of bilateral pneumothorax with mediastinal emphysema with interstitial emphysema.There were primary lung diseases in 32 cases:aspiration syndrome,neonatal pneumonia,neonatal respiratory distress syndrome and wet lung.Conclusion:The diagnostic rate of low-dose CT for complicated neonatal airleak syndrome is 100%,and it is more valuable for the diag-nosis of primary pulmonary diseases.
