腹腔镜联合醋酸戈舍瑞林治疗子宫内膜异位症的临床疗效分析

摘　　要：目的探讨腹腔镜联合醋酸戈舍瑞林治疗子宫内膜异位症的临床疗效及对血清性激素的影响。方法收集2017年1月至2018年1月在肇庆市第一人民医院妇产科收治的因子宫内膜异位症而行手术治疗的60例患者作为研究对象,并随机分为观察组(n=30)和对照组(n=30),对照组单纯性腹腔镜手术治疗,观察组患者在以上腹腔镜手术治疗基础上配合醋酸戈舍瑞林进行治疗,观察比较两组患者的临床疗效、血清性激素水平及复发率。结果观察组患者临床治疗总有效率(90.00%)明显高于对照组(66.67%),差异有统计学意义(χ^(2)=4.81,P=0.03);观察组患者治疗后雌激素(E2)、促卵泡生成素(FSH)、促黄体生成素(LH)水平均较对照组治疗后明显降低(t值分别为84.28,43.04,23.11,均P<0.05);观察组患者治疗后E2、FSH、LH水平较治疗前明显降低(t值分别为85.48,31.12,22.82,P<0.05);对照组患者治疗后FSH和LH水平均较治疗前降低(t=3.54,P<0.01;t=23.15,P<0.01),对照组患者E2水平在治疗前后均无统计学差异(t=0.35,P=0.73);对照组在6个月内、6~<12个月以及12~18个月的复发率均较观察组高(χ^(2)值分别为5.19、7.68、5.46,P<0.05)。结论腹腔镜联合醋酸戈舍瑞林治疗子宫内膜异位症的疗效确切,且可以降低血清性激素水平,值得临床推广应用。Objective To investigate the clinical efficacy of laparoscopy combined with goserelin acetate in the treatment for endometriosis and its effect on serum sex hormones.Methods 60 patients,who were admitted to the Department of Obstetrics and Gynecology of the first people′s hospital of Zhaoqing city from January 2017 to January 2018 and underwent surgical treatment for endometriosis,were selected as the research objects.They were randomly divided into the observation group(n=30)and the control group(n=30).The control group was treated with simple laparoscopic surgery,and the observation group was treated with goserelin acetate on the basis of the above laparoscopic surgery.The clinical efficacy,levels of serum sex hormones,and recurrence rates of the two groups were observed and compared.Results The total effective rate of clinical treatment in the observation group(90.00%)was obviously higher than that of the control group(66.67%),and the difference was statistically significant(χ^(2)=4.81,P=0.03).The levels of estradiol(E2),follicle-stimulating hormone(FSH),and luteinizing hormone(LH)in the observation group after treatment were all lower than those in the control group after treatment(t=84.28,43.04 and 23.11,respectively,P<0.05).The levels of E2,FSH and LH in the observation group after treatment were obviously lower than those before treatment(t=85.48,31.12 and 22.82,respectively,P<0.05).The level of FSH and LH of the control group patients after treatment was lower than that before the treatment(t=3.54,P<0.01;t=23.15,P<0.01),and the E2 levels of the control group patients were not statistically different before and after treatment(t=0.35,P=0.73).The recurrence rates of the control group within 6 months,6~<12 months and 12~18 months were higher than those of the observation group(χ^(2)=5.19,7.68 and 5.46,respectively,P<0.05).Conclusion Laparoscopy combined with goserelin acetate has a definite effect on the treatment for endometriosis,and can reduce the levels of serum sex hormones,which is worthy of clinic
