摘 要:目的调查山东汉族人群HLA-B*58:01、HLA-B*15:02和HLA-A*31:01等位基因携带率和等位基因频率,指导患者临床用药,并评估进行临床基因筛查的必要性。方法调查山东省骨髓库具有HLA-A、HLA-B位点高分辨数据的汉族人群样本,统计HLA-B*58:01、HLA-B*15:02和HLA-A*31:01等位基因携带率和等位基因频率,并按照志愿者籍贯进行分组,归为14个地市。结果山东汉族人群中,HLA-B*58:01、HLA-B*15:02、HLA-A*31:01等位基因携带率分别为8.65%、3.03%和7.3%。日照、东营、临沂地区HLA-B*58:01等位基因携带率达10%以上;枣庄、滨州和济南地区HLA-A*31:01等位基因携带率达8%以上;聊城、济南、淄博地区HLA-B*15:02等位基因携带率分别为3.96%、3.84%和3.77%。结论山东地区汉族人群患者在使用别嘌醇和卡马西平药物之前进行HLA-B*58:01和HLA-A*31:01基因筛查是有必要的,HLA-B*15:02等位基因携带率虽较低,但有条件的地区可以在开展用药前进行基因筛查,以减少严重药物不良反应的发生。Objective To investigate the allele carring rates and allele frequencies of HLA-B*58:01,HLA-B*15:02 and HLA-A*31:01 in Shandong Han population,guide the clinical medication of patients and evaluate the necessity of clinical gene screening.Methods The samples of Shandong Han population with high-resolution data of HLA-A and B loci in the bone marrow bank of Shandong Province were investigated,and the allele carring rates and allele frequencies of HLA-B*58:01,HLA-B*15:02 and HLA-A*31:01 were calculated.The results were grouped and divided into 14 cities according to the native places of volunteers.Results The allele carring rates of HLA-B*58:01,HLA-B*15:02 and HLA-A*31:01 were 8.65%,3.03%and 7.3%,respectively,in Shandong Han population.The allele carring rates of HLA-B*58:01 in Rizhao,Dongying and Linyi regions were more than 10%,and those of HLA-A*31:01 in Zaozhuang,Binzhou and Jinan regions were more than 8%.The allele carring rates of HLA-B*15:02 in Liaocheng,Jinan and Zibo regions were 3.96%,3.84%and 3.77%,respectively.Conclusion It is necessary to perform the genetic screening of HLA-B*58:01 and HLA-A*31:01 for the patients in Shandong Han population before using allopurinol and carbamazepine.Although the allele carring rate of HLA-B*15:02 is low,the gene screening before medication should be carried out in the conditional area to reduce the occurrence of serious adverse drug reactions.
