输尿管镜下钬激光治疗输尿管结石合并糖尿病的临床疗效和安全性观察

摘　　要：目的观察对输尿管结石合并糖尿病患者使用输尿管镜下钬激光治疗的安全性与临床疗效。方法选取2018年4月至2019年10月在本院接受治疗的输尿管结石合并糖尿病患者78例作为研究对象,随机分为对照组(传统开腹手术治疗)和实验组(输尿管镜下钬激光碎石术治疗),各39例。比较两组患者手术指标、临床疗效、结石排净情况及并发症发生情况。结果实验组患者治疗总有效率、结石排净率均高于对照组(P<0.05);实验组手术操作时间、术后下床活动时间、肠胃功能恢复时间、住院时间均短于对照组,术中出血量少于对照组(P<0.05);实验组并发症发生率低于对照组(P<0.05)。结论输尿管镜下钬激光治疗输尿管结石合并糖尿病患者效果显著,可提高结石排出率,且术后并发症发生率较低,可促进患者早日康复出院,值得临床推广运用。Objective To observe the safety and clinical efficacy of ureteroscopic holmium laser in patients with ureteral calculi complicated with diabetes.Methods 78 patients with ureteral calculi complicated with diabetes who received treatment in our hospital from April 2018 to Octo-ber 2019 were selected as the study subjects,and they were randomly divided into control group(traditional open surgery)and experimental group(holmium laser lithotripsy under ureteroscopy),with 39 cases in each group.The surgical indicators,clinical efficacy,stone removal and complica-tions were compared between the two groups.Results The total effective rate and stone removal rate in the experimental group were higher than those in the control group(P<0.05).The operative time,postoperative ambulation time,gastrointestinal function recovery time,hospital stay time of the experimental group were shorter than the control group,and the amount of intraoperative blood loss was less than the control group(P<0.05).The complication rate of experimental group was lower than that of control group(P<0.05).Conclusion Holmium laser under ureteroscopy has a significant effect on ureteral calculi combined with diabetes,which can improve the stone excretion rate,and the incidence of postoperative complica-tions is low.It can promote the early recovery and discharge of patients,which is worthy of clinical application.
