摘 要:目的评价三维全自动左心室容积定量技术(HM)检测冠状动脉粥样硬化性心脏病(简称冠心病)患者左心收缩功能的可行性。方法随机选取2019年6月至2020年6月住院的冠心病患者100例,分为冠心病心梗组(A组,60例),冠心病非心梗组(B组,40例),同期匹配正常对照组(C组,50例),用HM法和双平面Simpson’s法测量左心室舒张末容积(LVEDV)、左心室收缩末容积(LVESV)、左心室射血分数(LVEF),记录时间,并与实时三维超声(RT-3DE)测值比较分析。结果HM法LVEDV、LVESV均高于RT-3DE法测值(均P<0.05),LVEF无统计学差异。A、B组HM法及RT-3DE法LVEF低于双平面Simpson’s法测值。HM法A组LVEDV、LVESV高于B、C组,LVEF低于B、C组(均P<0.05)。HM法LVEDV、LVESV、LVEF测值与RT-3DE法相关性高于双平面Simpson’s法,一致性好。HM法用时短,LVEDV、LVESV、LVEF组间及组内相关系数(ICC)高。结论HM是一项可重复性高、客观性强、能更快速准确评估冠心病患者左心收缩功能的技术。Objective To evaluate the application value of three-dimensional fully automated left ventricular volume quantification technique(HeartModel)in left heart systolic function among patients with coronary heart disease.Methods A total of 100 inpatients with coronary heart disease were randomly selected between June 2019 and June 2020.Patients were divided into the myocardial infarction group(group A)(60 patients),and the non-myocardial infarction group(group B)(40 patients).Simultaneously,50 people were selected as healthy control subjects(group C).Left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),and left ventricular ejection fraction(LVEF)were measured using the Simpson’s method and HM(HeartModel)and recording time,comparing with the results of real-time three-dimensional echocardiography(RT-3DE).Results LVEDV and LVESV measured by HM were higher than those measured using the RT-3DE method(all P<0.05).LVEF showed no significant difference.The LVEF of HM and RT-3DE in patients with coronary heart disease was lower than that of Simpson’s method.The LVEDV and LVESV of HM in group A were higher than those of group B and C,while LVEF was lower(all P<0.05).The correlation among LVEDV,LVESV,and LVEF measured using HM and RT-3DE was higher than that of Simpson’s method and the consistency was within clinical acceptance.The time of HM was significantly less.The intra-and inter-correlation coefficients(ICC)for LVEDV,LVESV,and LVEF values with HM were higher.Conclusion HM is a highly repeatable,objective,rapid,and accurate technique for left heart systolic function in patients with coronary heart disease.
