我院信息化手段构建药学服务生态圈的实践

摘　　要：目的:用信息化手段提供创新型"互联网+药学服务"。方法:整理我院2010-2019年药学服务记录,分析查找临床高频问题,用信息化手段构建药学知识库,对药学服务模式进行创新性探索。结果:建成内容涵盖药品基本信息、药学监护要点、用药注意事项、药物相互作用、儿科用药建议、药物配伍禁忌等方面的反馈更新型知识库。并以此为基础,发布微信小程序"药觅索",构建"医-护-药"三方的"互联网+药学服务"生态圈。结论:"互联网+药学服务"生态圈的成功建立,使我院的药学服务更具针对性和时效性,为药师融入临床治疗团队创造了机会,也为医院药学在新形式下的发展探索出一条新路。Objective:To provide innovative“Internet+Pharmaceutical Care”by information technology.Methods:Pharmaceutical care records of our hospital from 2010 to 2019 were analyzed and searched for clinical high-frequency problems,the pharmaceutical care knowledge database was constructed by information technology,and the pharmaceutical care model was innovatively explored.Results:The new feedback knowledge database was constructed with basic drug information,key points of pharmaceutical care,medication precautions,drug interactions,pediatric medication recommendations,drug incompatibility and etc.On this basis,the Wechat advanced placement program“Yaomisuo”was launched,and“Internet+Pharmaceutical Care”ecosystem with“Clinician-Nurse-Pharmacist”was constructed.Conclusion:The construction of“Internet+Pharmaceutical Care”ecosystem makes the pharmaceutical care of our hospital more targeted and time-based,creates opportunities for pharmacists to integrate into the clinical treatment team,and explores the new approach for the development of hospital pharmacy.
