摘 要:目的:研究孕11-14周经彩超测定子宫动脉血流频谱预测子痫前期的临床价值。方法:选择我院2019年10月-2020年7月我院经彩超测定子宫动脉血流频谱的1290例产妇,将其中子宫动脉血流测值异常经诊断为子痫前期的13例产妇作为研究组,另选取于我院健康体检的产妇13例作为正常组,所有受检者均接受彩超检查,对比两组孕妇及子痫前期的经产妇、初产妇孕11-14周的阻力指数(RI)、搏动指数(PI)及胎儿脐动脉血流(S/D)的差异。结果:研究组孕11周PI值>12周>13周>14周,差异具有统计学意义(P<0.05);研究组孕11、12、13、14周的PI值均>正常组,差异具有统计学意义(P<0.05);初产妇和经产妇的PI、RI以及S/D值差异无统计学意义(P>0.05)。结论:彩超测定子宫动脉血流频谱可早期预测孕11-14周孕妇的子痫前期,为临床治疗和干预提供有效诊断依据。Objective:To study the clinical value of uterine artery blood flow spectrum measured by color Doppler ultrasound in predicting preeclampsia from 11-14 weeks gestation.Methods:From October 2019 to July 2020 in our hospital,1290 pregnant women whose uterine artery blood flow spectrum was measured by color Doppler ultrasound were selected.Among them,13 pregnant women with abnormal uterine artery blood flow were diagnosed as preeclampsia as the research group.In addition,13 parturients who received a physical examination in our hospital were selected as the normal group.All subjects received color Doppler ultrasound examination to compare the resistance index(RI)of the two groups of pregnant women,preeclampsia,and primiparous women at 11-14 weeks of pregnancy.,Pulsatility index(PI)and fetal umbilical artery blood flow(S/D)difference.Results:The PI value of the study group at 11 weeks of pregnancy>12 weeks>13 weeks>14 weeks,the difference was statistically significant(P<0.05);the PI values of the study group at 11,12,13,and 14 weeks of pregnancy were all>the normal group,the difference was significant Statistically significant(P<0.05);PI,RI and S/D values of primipara and postpartum women were not statistically significant(P>0.05).Conclusion:The color Doppler ultrasound measurement of the uterine artery blood flow spectrum can predict the preeclampsia of pregnant women at 11-14 weeks gestation early,and provide effective diagnostic evidence for clinical treatment and intervention.
