基于质控图联合Westgard多规则理论的替考拉宁血药浓度监测的室内质控评估

摘　　要：目的应用质控图联合Westgard多规则法评估替考拉宁血药浓度监测的室内质量控制,提高治疗药物监测质量。方法回顾性统计2019年1-6月62组替考拉宁高、中、低浓度的随行质控品的实测浓度,分别绘制Levey-Jennings质控图和Z-分数质控图,采用Westgard多规则理论进行室内质量控制的评估。结果替考拉宁低、中、高室内质控的日内和日间RSD分别为1.92%~3.81%和2.84%~3.83%,回收率分别为105.42%~106.97%和102.98%~105.33%,符合生物样品检测的要求。绘制的Levery-Jennings质控图和Z-分数质控图通过Westgard多规则法评估显示,低、中、高浓度质控血浆样品均未出现失控,替考拉宁治疗药物监测结果准确可靠。结论建立的替考拉宁血药浓度监测室内质量控制体系,具有较好的精密度和准确性,对提高临床用药的安全合理性具有重要意义。Objective To improve the quality of therapeutic drug monitoring by evaluating the indoor quality control of teicoplanin blood concentration monitoring with Westgard multi-rule theory based on the quality control chart.Methods The actual concentrations of 62 groups of accompanying quality control samples with high,medium and low concentrations of teicoplanin from January to June in 2019 were retrospectively counted.The indoor quality control was evaluated by Westgard multi-rule theory based on analyzing Levey-Jennings quality control charts and Z-score quality control chart.Results The intra-day RSD of teicoplanin was 1.92%~3.81%,the inter-day RSD was 2.84~3.83%,and the recovery rates were respectively 105.42%~106.97%and 102.98%~105.33%,which met the requirements for biological sample determination.Levey-Jennings control charts and Z-score control chart were evaluated by Westgard multi-rule theory,which showed that there was no loss of control in low,medium,or high-concentration quality control plasma samples,and the results of teicoplanin were accurate and reliable.Conclusion The indoor quality control system of monitoring of teicoplanin blood concentration is of good precision and accuracy,which is of great significance to improve the safety and rationality of clinical medication.
