摘  要:慢性淋巴细胞白血病(CLL)患者由于存在免疫功能紊乱,常发生自身免疫性血细胞减少,免疫性血小板减少症(ITP)是其中常见的合并症之一。CLL相关ITP的发病机制复杂,尚未完全阐明,目前研究主要针对体液免疫、细胞免疫及固有免疫失调等方面。近年来研究结果提示,基因组学的异常和micro RNA也参与了CLL相关ITP的发生。传统的ITP标准治疗对CLL相关ITP疗效欠佳,针对CLL原发病的化疗或单克隆抗体治疗能够纠正治疗血小板减少,新的靶向药物的出现为该病治疗提供了新的选择。本文就近年来针对CLL患者相关ITP的发病机制、预后及治疗的最新研究进展作一综述。Chronic lymphocytic leukemia(CLL)patients usually show immune dysfunction,which often leads to autoimmune hemocytopenia.Immune thrombocytopenia(ITP)is one of the common complications.The pathogenesis of CLL-related ITP is complex and has not been fully elucidated.At present,the researches mainly focus on humoral immunity,cellular immunity and innate immune disorders.Recent studies suggest that genomic abnormalities and microRNAs are also involved in CLL-related ITP.Traditional ITP standard therapy has a poor effect on CLL-related ITP.Chemotherapy or monoclonal antibody therapy against the primary pathogenesis of CLL can effectively treat thrombocytopenia,and the emergence of new targeted drugs also provides new treatment options for the disease.In this paper,the progresses of CLL-related ITP pathogenesis,prognosis and treatment in recent years are reviewed.
