摘  要:目的探讨2型糖尿病(T2DM)患者血清维生素E结合蛋白(AFM)和锌-α2-糖蛋白(ZAG)的水平变化及其与高血压发病的相关性。方法选取2018年12月~2020年1月保定市第一中心医院内分泌科收治的T2DM患者147例作为研究对象。将其按照血压水平不同分为单纯T2DM组(DS组)75例,T2DM合并高血压组(DH组)72例。另选同期性别、年龄相匹配的健康体检者70例作为对照组(NC组)。收集所有受试者的一般资料并进行常规生化检测,采用酶联免疫吸附法(ELISA)法检测AFM和ZAG水平,利用Spearman相关分析及多元线性回归法分析AFM和ZAG的相关因素。结果与NC组相比,DS组与DH组AFM水平明显升高,以DH组最高(P<0.05);DS组、DH组ZAG水平显著降低,以DH组最低(P<0.05)。Spearman相关分析显示血清AFM水平与BMI、WHR、DM病程、SBP、DBP、TC、FPG、HbA1c、FINS、HOMA-IR呈正相关,与ZAG呈负相关(P<0.01)。血清ZAG水平与BMI、WHR、DM病程、SBP、DBP、TC、FPG、HbA1c、FINS、HOMA-IR、AFM呈负相关(P<0.01)。多元线性回归分析显示SBP、FPG、HOMA-IR、ZAG为血清AFM水平的独立影响因素;SBP、FPG、AFM是ZAG的独立影响因素。结论随着2型糖尿病患者血压的升高,血清维生素E结合蛋白水平显著升高,锌-α2-糖蛋白水平明显降低,二者的变化可能在2型糖尿病合并高血压的病程进展中产生一定影响。Objective To clarify the changes of serum afamin(AFM)protein and zinc-α2-glycoprote(ZAG)in type 2 diabetes mellitus(T2DM)patients and their correlation with hypertension.Methods A total of 147 patients with T2DM in this study included 74 males and 73 females.The subjects were classified into simple diabetes group(DS,n=75)and diabetes complicated with hypertension group(DH,n=72)according to different blood pressure levels.Simultaneously 70 healthy subjects whose age and gender matched the experimental groups in the same period were selected as normal control group(NC group).General data of all individuals were collected and routine biochemical tests were conducted.The serum levels of AFM and ZAG were detected by ELISA.Spearman correlation analysis and multivariate regression analysis were utilized to assess the impact factors of AFM and ZAG,and receiver operating characteristic(ROC)curve analysis were used to analysis the value of the two indicators in evaluation the risk factors of T2DM combined with hypertension.Results Compared with NC group,AFM levels in DS group and DH group were significantly higher than that in NC group,with DH group being the highest(P<0.05).ZAG concentrations were gradually descended in each group,and the lowest in DH group(P<0.05).Spearman correlation analysis showed that serum AFM level was positively correlated with BMI,WHR,duration of DM,SBP,DBP,TC,FPG,HbA1c,FINS and HOMA-IR,and negatively correlated with ZAG(P<0.01).While serum ZAG level was negatively correlated with BMI,WHR,duration of DM,SBP,DBP,TC,FPG,HbA1c,FINS,HOMA-IR and AFM(P<0.01).Multivariate linear regression analysis showed that SBP,FPG,HOMA-IR and ZAG were independent influencing factors for AFM level.SBP,FPG and AFM were independently correlated with ZAG.Conclusion With the ascent of blood pressure in T2DM patients,the serum AFM concentrations were obviously up-regulated,while the serum ZAG levels were distinctly down-regulated.These changes of the two biomarkers may exert some influence in the course of T2DM with hype
