云南玉溪彝族与汉族患者舒芬太尼镇痛效果差异的比较研究

摘　　要：目的比较玉溪地区彝族与汉族患者术后早期舒芬太尼镇痛效果的差异性。方法择期行腹腔镜下胆囊切除术的病例254例,按民族分为彝族组(Y组,n=127)和汉族组(H组,n=127)。麻醉诱导使用舒芬太尼0.4ug/kg,术中泵入丙泊酚及瑞芬太尼维持麻醉,于放气腹前静注舒芬太尼0.1ug/kg,患者入复苏室后行视觉疼痛评分(VAS),对VAS≥4分的患者每次追加舒芬太尼0.05ug/kg至VAS<4分。记录患者最大VAS评分及舒芬太尼追加次数、不良反应及复苏室停留时间。结果Y组患者在复苏室最大VAS评分高于H组,镇痛次数比例大于H组,2组间有统计学意义(P<0.05);H组在复苏室恶心呕吐发生率大于Y组(P<0.05)。结论云南玉溪彝族比汉族患者在复苏室需要更多舒芬太尼镇痛,临床用药应考虑种族因素,个体化用药。Objective To compare the analgesic effect difference of sufentanil in the early postoperative period between Yi and Han patients Yunnan Province.Methods 254 cases of laparoscopic cholecystectomy were performed in Yi group(Y group,n=127)and Han group(H group,n=127).Sufentanil 0.4ug/kg was used for induction of anesthesia,pumped propofol and remifentanil to keep anesthesia intraoperation,and 0.1ug/kg sufentanil was injected intravenously before releasing pneumoperitoneum.The visual analogue scale(VAS)was performed after the patient entered the resuscitation room.Sufentanil 0.05ug/kg-1time-1 was added to patients with VAS≥4 points and stopped to VAS score<4 points.It recorded the patient's maximum VAS score,the number of additional of sufentanil,adverse reactions and stay time in the resuscitation room.Results The maximum VAS score in the resuscitation room of group Y was higher than that of group H,the analgesic ratio was more than that of group H,the incidence of nausea and vomiting in the resuscitation room in group H was more than that in group Y(P<0.05).Conclusions The Yi people need more sufentanil for analgesia in the resuscitation room than the Han people.The clinical medication should consider ethnic factors and individualized medication.
