摘 要:目的利用磁共振水脂分离成像技术(mDIXON)测量旁矢状位腰椎椎体的脂肪分数,分析不同骨量组腰椎椎体脂肪含量的变化,并评价其诊断效能。方法纳入49例腰椎检查患者,所有患者均行DXA检查及腰椎MR检查。以DXA腰椎椎体T值为金标准,将患者划分为骨量正常、骨量减低和骨质疏松三组。分别测量L1~L4椎体左旁矢状位和右旁矢状位的椎体脂肪分数(FF)值。对不同骨量组的腰椎椎体FF值进行比较,分析腰椎FF值与骨质疏松程度的相关性,并评估其诊断效能。结果L3椎体右旁矢状位和L4椎体左旁矢状位的FF值与患者骨质疏松程度呈正相关(r=0.321,0.330,P=0.024,0.021)。L1椎体右旁矢状位、L3椎体右旁矢状位和L4椎体左旁矢状位FF值对骨质疏松具有较好的诊断效能(AUC=0.661~0.696)。结论随着骨质疏松的发生发展,腰椎旁矢状位的FF值整体呈增高趋势;L1~L4椎体旁矢状位脂肪分数在早期预测骨质疏松中具有潜在应用价值。Objective Measure the fat fraction of lateral sagittallumbar vertebrae using magnetic resonance water and fat separation imaging technology(mDIXON),to analyze the change of fat content in the lumbar vertebra in different bone mass groups,and evaluate its diagnostic efficacy.Methods Forty-nine cases with lumbar spine examination were enrolled.All patients underwent DXA and MR examination of lumbar spine.The T value of lumbar vertebral body derived from DXA was used as the gold standard,and the patients were divided into three groups:normal group,osteopenia group and osteoporosis group.The fat fraction(FF)values of the left lateral sagittal and right lateral sagittal planes of the L1~L4 vertebral bodies were measured respectively.The FF values of lumbar vertebrae in different bone mass groups were compared.The correlation between the lumbar-vertebra FF value and degree of osteoporosis was analyzed,and its diagnostic efficacy was evaluated.Results The FF values of the L3 vertebral-body right lateral sagittal plane and the L4 vertebral-body left lateral sagittal plane were positively correlated with degree of osteoporosis(r=0.321,0.330;P=0.024,0.021).The FF values of the L1 vertebral-body right lateral sagittal plane,L3 vertebral-body right lateral sagittal plane and L4 vertebral-body left lateral sagittal FF value has a good diagnostic efficiency for osteoporosis(AUC=0.661~0.696).Conclusion With the increasing degree of osteoporosis,the FF value of lateral sagittallumbar vertebrae was on the rise in total.The FF value of lateral sagittal lumbar vertebrae held great potential in the early prediction of osteoporosis.
