摘　　要：目的对某院住院药房的干预处方进行分析,分类评价不合理处方,促进临床的合理用药,提高临床用药的准确性。方法通过收集某院“住院部口服药房”2019年11月~2020年10月共12个月的干预处方,参照《处方管理办法》进行分析,药师从超说明书用药、人工误录入、配伍禁忌、用法用量等方面,对处方的合理性进行审核,评价不合理处方,并对不合理处方进行有效的干预,继而提升合理用药准确性。结果通过科室不合理处方计数以及处方差错原因、处方不合理类型等进行分析,并以此为依据作出了一系列减少处方差错的对策。结论加强对处方的审查工作能有效减少不合理用药,提高处方合格率,保障医疗安全。Objective To analyze the intervention prescriptions of inpatient pharmacy in a hospital,classify and evaluate the unreasonable prescriptions,promote rational clinical drug use,and improve the accuracy of clinical drug use.Methods By collecting the intervention prescriptions of the inpatient oral pharmacy of the hospital from November 2019 to October 2020 with reference to the Prescription Management Method,pharmacist can verify the rationality of the prescriptions,evaluate the unreasonable prescriptions,and effectively intervene the unreasonable prescriptions by considering off-label drug use,artificial false entry,compatibility taboo,usage,dosage,etc.,so as to improve the accuracy of rational drug use.Results Analyzed the counts of unreasonable prescriptions in the department,the causes of prescription errors,and the types of unreasonable prescriptions.Based on this,a series of countermeasures were made to reduce prescription errors.Conclusion。
