摘　　要：目的通过DRGs指标运用对某医院的妇科系统医疗服务能力进行综合评价,为不断提高医院医疗服务水平,促进医院绩效精细化管理提供依据。方法基于CHS-DRG乌鲁木齐本地化版本分组器对新疆某专科医院2017-2019年妇科系统22276例病例的分组结果,从服务能力、服务效率和医疗安全3个维度以及主干病种指标分析对妇科系统的6个科室进行绩效评价。结果综合指数位列第一的是D科室为5.12,最后一名是F科室为4.27。科室B和科室D的费用消耗指数及时间消耗指数均小于1,服务效率较其他科室高。妇科系统主干病种中,NA19女性生殖器官恶性肿瘤的广泛切除手术组CMI值最大为3.86,费用权重第二高为3.9328。结论采用DRGs方法评价临床科室在疾病诊治过程中的“专病专治”的程度、各科室的医疗服务绩效评价及其各科室间的横向比较,能评价临床专科在某地区的诊治水平。Objective Through the application of DRGs index,the ability of the medical service of gynecological system in some hospital was comprehensively assessed,which provides a basis for continuously improving the level of medical service of hospital and promoting the fine management of hospital performance.Methods Based on the CHS-DRG Urumqi local version grouping results of 22,276 cases in the gynecological system of Xinjiang Medical University Af⁃filiated Cancer Hospital from 2017 to 2019,the performance of six departments in the gynecological system was evalu⁃ated from three dimensions of service ability,service efficiency and medical safety,as well as the index of main diseas⁃es.Results In the overall index,D department ranked first with 5.12,and F department ranked last with 4.27.The cost consumption index and time consumption index of department B and Department D are both less than 1,and their ser⁃vice efficiency is higher than other departments.Among the main disease of department of gynaecological system,the CMI value of NA19 female reproductive organs wide resection of malignant tumor treatment was up to 3.86,the second highest charge weight was 3.9328.Conclusion Using DRGs method to evaluate the degree of“special treatment”in the process of disease diagnosis and treatment in clinical departments,the medical service performance evaluation of each department and the horizontal comparison among different departments can evaluate the diagnosis and treat⁃ment level of clinical department in a certain area.
