许昌市2015—2019年儿童急性弛缓性麻痹病例监测结果分析

摘　　要：目的分析许昌市15岁以下儿童急性弛缓性麻痹(AFP)的流行病学特征,了解AFP病例报告敏感性,评价AFP病例监测工作质量。方法对许昌市2015—2019年15岁以下儿童AFP病例监测系统报告的AFP病例进行描述性分析。结果许昌市2015—2019年共报告15岁以下AFP病例161例,年平均报告发病率为3.63/10万,全市AFP病例报告发病率最高为鄢陵县(4.96/10万),其次为魏都区(4.58/10万);春季、夏秋季1~2岁组发病率最高,以散居儿童为主,男女性别比为1.61:1;首诊漏报率随就诊医疗机构级别的降低有所升高,首诊漏报病例集中在县、乡、村级医疗机构,漏报率之间比较差异有显著统计学意义(P<0.01)。结论许昌市AFP病例监测系统运行良好,达到国家监测指标要求,需要提高基层病例报告敏感度以及诊疗水平,降低首诊病例漏报率。Objective To analyze the epidemiological characteristics of acute flaccid paralysis(AFP)in children under 15 years old in Xuchang,and understand the sensitivity of AFP case reporting and evaluate the quality of AFP case surveillance.Methods A descriptive analysis of AFP cases reported by the AFP case surveillance system in children under 15 years old in Xuchang from 2015 to 2019 was conducted.Results From 2015 to 2019,a total of 161 AFP cases under the age of 15 were reported in Xuchang,with an average annual incidence of 3.63 per 100000,with the highest incidence of 4.96 per 100000 in Yanling County,followed by 4.58 per 100000 in Weidu District.In spring and summer and autumn,the incidence of 1-2-year-old children was the highest,and the ratio of male to female was 1.61:1.The first missed cases were concentrated in county,township and village level medical institutions,and there was significant difference in the rate of missed cases(P<0.01).Conclusion The AFP case surveillance system in Xuchang is working well and can meet the requirements of national surveillance index.It is necessary to improve the sensitivity of primary case reporting and the level of diagnosis and treatment,so as to reduce the rate of missing report of primary cases.
