摘　　要：目的:探讨肺部超声膈肌运动指标对脓毒症休克患者呼吸机撤离的指导作用。方法:以本院2020年4月—2021年2月收治的50例脓毒症休克机械通气患者为对象,分为撤机成功组和失败组。所有患者均接受膈肌超声检查,分析超声参数预测呼吸机撤离的价值。结果:成功组机械通气时间、住院时间及病死率均显著低于失败组(P<0.05)。成功组膈肌位移显著高于撤机失败组,D-RSBI、RSBI水平显著低于失败组(P<0.05)。膈肌位移、D-RSBI、RSBI预测撤机失败的AUC分别为0.836、0.800、0.730。结论:床旁超声监测膈肌运动参数膈肌位移及D-RSBI可有效预测撤机失败。Objective To explore the guiding role of pulmonary ultrasound diaphragm movement index in ventilator evacuation of septic shock patients.Methods From April 2020 to February 2021,50 patients with septic shock undergoing mechanical ventilation were divided into two groups:successful weaning group and failure group.All patients underwent diaphragmatic ultrasound examination to analyze the value of ultrasound parameters in predicting ventilator withdrawal.Results The duration of mechanical ventilation,length of hospital stay and mortality in the successful group were significantly lower than those in the failed group(P<0.05).The diaphragmatic displacement of successful group was higher significantly than that of failed group,and the levels of d-rsbi and RSBI were significantly lower than those of failed weaning group(P<0.05).The AUC of diaphragm displacement,d-rsbi and RSBI for predicting weaning failure were 0.836,0.800 and 0.730,respectively.Conclusion Bedside ultrasound monitoring diaphragmatic motion parameters,diaphragmatic displacement and d-rsbi can effectively predict weaning failure.
