隐匿性胫骨平台骨折的X线征象分析

摘　　要：目的:回顾性分析隐匿性胫骨平台骨折(TPOF)X线征象及其临床应用价值。方法:选择2018年1月—2021年1月于我院急诊外科及骨科就诊的下肢外伤且经膝关节X线平片诊断为TPOF或疑似TPOF患者80例,入组患者均于X线平片检查后行膝关节MR检查以明确诊断,依据MR检查结果,将80例患者分为TPOF阳性组(52/80)和TPOF阴性组(28/80),并对膝关节X线相关征象进行归纳分析。结果:TPOF阳性患者X线检查提示关节面不规则75%(39/52),关节面增宽46.15%(24/52),胫骨近端骨小梁紊乱78.85%(41/52),均高于TPOF阴性组,差异有统计学意义(P<0.05),髌上囊肿胀显示率44.23%(23/52),与TPOF阴性组无明显统计学差异(P>0.05)。结论:X线平片作为膝关节外伤常规检查方法,需密切观察隐匿性胫骨平台骨折相关征象,提高检出率。Objective To retrospectively analyze the X-ray signs and clinical value of occult tibial plateau fracture(tpof).Methods From January 2018 to January 2021,80 patients with lower extremity trauma who were diagnosed as tpof or suspected tpof by knee X-ray plain film in our hospital were selected.All patients underwent knee MR examination after X-ray plain film examination to make a clear diagnosis.According to the results of MR examination,80 patients were divided into tpof positive group(52/80)and tpof negative group(28/80),and the X-ray correlation of knee joint was analyzed Signs were analyzed.Results X-ray examination of tpof positive patients showed 75%(39/52)of irregular articular surface,46.15%(24/52)of widened articular surface,78.85%(41/52)of trabecular disorder in proximal tibia,which were significantly higher than those of tpof negative group(P<0.05),and 44.23%(23/52)of swelling of suprapatellar capsule showed no significant difference with tpof negative group(P>0.05).Conclusion As a routine examination method of knee joint trauma,X-ray plain film should closely observe the related signs of occult tibial plateau fracture to improve the detection rate.
