腹膜透析病人自尊水平在病耻感与生活质量之间的中介效应分析

摘　　要：目的:探讨腹膜透析病人自尊水平在病耻感与生活质量之间的中介作用。方法:选取2020年7月—2021年1月在南宁市某三级甲等医院腹膜透析中心就诊的201例腹膜透析病人为研究对象,采用一般资料调查表、自尊量表(SES)、社会影响量表(SIS)、腹膜透析生活质量量表(PDQOLS)进行问卷调查,分析腹膜透析病人病耻感、自尊水平及生活质量的相关性,并对自尊水平在病耻感与生活质量之间的中介作用进行分析。结果:腹膜透析病人SES得分为(28.67±4.22)分、SIS得分为(58.27±11.34)分、PDQOLS得分为(73.57±10.20)分;病耻感与自尊水平呈负相关(r=-0.442,P<0.001),与生活质量呈负相关(r=-0.503,P<0.001),自尊水平与生活质量呈正相关(r=0.484,P<0.001);自尊水平在病耻感与生活质量之间存在部分中介效应,中介效应量为28.47%。结论:腹膜透析病人自尊水平、病耻感处于中等水平,生活质量有待改善,自尊水平作为中介变量对病人的生活质量产生影响。Objective:To explore the mediating role of self-esteem between stigma and quality of life(QOL).Methods:A total of 201 peritoneal dialysis(PD)patients who were treated in the PD center of a tertiary grade⁃A hospital in Nanning city from July 2020 to January 2021,were selected as the research subjects.Using General Information Questionnaire,Self⁃Esteem Scale(SES),Social Impact Scale(SIS),and Peritoneal Dialysis Quality of Life Scale(PDQOLS)to analyze the correlation between stigma,self⁃esteem,and QOL in PD patients.To analyze mediating role of self⁃esteem level between stigma and QOL in PD patients.Results:The SES score of PD patients was 28.67±4.22,the SIS score was 58.27±11.34,and the PDQOLS score was 73.57±10.20;stigma was negatively correlated with self⁃esteem(r=-0.442,P<0.001),negatively associated with QOL(r=-0.503,P<0.001),self⁃esteem level was positively correlated with QOL(r=0.484,P<0.001);self⁃esteem level played a partial mediating role between stigma and QOL,and the amount of mediation effect was 28.47%.Conclusions:The self⁃esteem and stigma of PD patients were at a moderate level,and the QOL might need to be improved.Self⁃esteem had an impact on the QOL of the patients as a mediating variable.
