摘　　要：目的探讨≤25岁子宫肌瘤患者肌瘤剔除术后复发的危险因素。方法回顾性分析2012年10月1日至2018年6月30日于北京协和医院行子宫肌瘤剔除术的患者134例。年龄≤25岁。记录患者一般信息及临床病理特征,采用非条件logistic回归模型,对肌瘤的复发因素进行分析。结果随访时间8~73个月,中位随访时间42.5个月。术后复发率13.2%,复发距手术的间隔为6~66个月,中位复发时间为24个月。子宫肌瘤家族史、术前促性腺激素释放激素激动剂(GnRH-a)治疗是复发的相关危险因素,差异有统计学意义(P<0.05);初潮年龄、BMI、临床症状以及肌瘤大小、数目、病理类型和手术方式与复发无相关性(P>0.05)。结论子宫肌瘤家族史及术前GnRH-a治疗是≤25岁子宫肌瘤患者肌瘤剔除术后复发的独立危险因素。Objective To analyze the postoperative recurrence risk factors of uterine leiomyoma after myomectomy among women 25 years old or younger.Methods A total of 134 patients aged 25 years old or younger were included for retrospective analysis,who underwent myomectomy in Gynecology Department of Peking Union Medical College Hospital from October 1,2012 to June 30,2018.Their general data and clinicopathological characters were recorded.The recurrence factors were analyzed by unconditional logistic regression model.Results The follow-up time ranged from 8 to 73 months with a median follow-up of 42.5 months.The recurrence rate was 13.2%.The interval between recurrence and operation was 6 to 66 months,and the median recurrence time was 24 months.The analysis showed that the family history of uterine leiomyoma and preoperative GnRH-a treatment were risk factors for recurrence,and there were significant differences(P<0.05).There were no statistic relationship between the recurrence and menarche age,BMI,clinical symptoms,or the size and number,pathological type of uterine leiomyoma or surgical approach(P>0.05).Conclusions The family history of uterine leiomyoma and preoperative GnRH-a treatment are independent risk factors for recurrence after myomectomy among women 25 years old or younger with uerine leiomyoma.
