筋膜手法松解联合动态关节松动术在颈源性头痛中的应用

摘　　要：目的:探讨筋膜手法松解联合动态关节松动术在颈源性头痛中的治疗应用和价值。方法:共纳入124例颈源性头痛患者,随机分为观察组和对照组各62例,对照组予以基础药物治疗,观察组在药物治疗基础上予以筋膜手法松解联合动态关节松动术治疗。通过视觉模拟评分法(VAS)、日本骨科协会评分法(JOA)评分和生活质量量表(SF-36)评分评估治疗前后疼痛缓解、颈椎功能和总体健康情况。结果:在疼痛缓解方面,两组患者在治疗后VAS评分均有所降低,观察组评分降低较对照组更显著(P<0.05)。颈椎功能JOA评分,观察组治疗后运动评分明显改善,且明显优于对照组(P<0.05),治疗后颈椎功能改善疗效观察组较对照组明显提高(P<0.05)。SF-36评分,观察组治疗后生理功能、生理职能、躯体疼痛、总体健康和社会功能方面均显著优于对照组(P<0.05),而精力、情感职能和心理健康差异无统计学意义(P>0.05)。结论:筋膜手法松解联合动态关节松动术可有效治疗颈源性头痛、改善症状,具有较好的应用价值。Objective:To study the therapeutic application and value of the manual fascial release combined with dynamic arthrolysis in the treatment of cervicogenic headache.Methods:A total of 124 patients with cervicogenic headache were recruited.All the patients were randomly divided into the experimental group(62 patients)and the control group(62 patients).The patients in the control group were treated with primary drug treatment,and the patients in the experimental group were treated with manual fascial release combined with dynamic arthrolysis on the basis of primary drug treatment.The VAS score,JOA score and SF-36 scale were used to evaluate the pain level,the function of cervical vertebra and total healthy condition of the both group before and after treatments.Results:In terms of pain relief,the VAS score of both groups reduced after treatment,and the VAS score of the experimental group was much improved compared with that of the control group(P<0.05).In terms of the cervical vertebra function evaluated by JOA score,the subclass score about motion function of the experimental group was markedly improved after treatment and was better than that of the control group(P<0.05).Besides,the improvement rate of the cervical vertebra function evaluated by JOA score of the experimental group was significantly higher than that of the control group(P<0.05).In terms of SF-36 scale score,the physiological function,physiological competency,pain level,total healthy level and social function of the experimental group were much better than those of the control group(P<0.05),and the vigor,emotion and mental health were comparable(P>0.05).Conclusion:Manual fascial release combined with dynamic arthrolysis is an effective treatment for cervicogenic headache and it can relieve the symptom of the patients.It has good application value.
