摘　　要：目的测定GDM患者体内镁离子,25-羟基维生素D3与IL-6水平,进而探讨镁离子及25-羟基维生素D3与IL-6及糖代谢指标的关系。方法选择2018年10月至2019年10月常规产前检查住院分娩的妊娠晚期、单胎孕妇共200例,其中GDM组100例,糖耐量正常(NGT)组100例。检测2组孕妇外周血血清镁、25-羟基维生素D3、IL-6、空腹血糖、空腹胰岛素、胰岛素抵抗指数、三酰甘油、总胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇水平,并分析血清镁、25-羟基维生素D3及IL-6与血糖、胰岛素抵抗指数及血脂水平的关系。结果GDM组TG、空腹血糖、胰岛素、胰岛素抵抗指数均明显高于NGT组(P<0.05)。GDM组及NGT组血清镁离子水平分别为[(0.56±0.07)ng/ml;(0.92±0.11)ng/ml],2组比较,GDM组镁离子水平明显低于NGT组(P<0.05);GDM组及NGT组血清25-羟基维生素D3水平分别为[(15.32±3.24)ng/ml;(21.47±5.38)ng/ml],2组比较,GDM组镁离子水平明显低于NGT组(P<0.05);GDM组和NGT组血清中IL-6水平分别为[(164.70±39.54)ng/ml;(98.21±28.91)ng/ml],2组比较,GDM组血清中IL-6水平明显高于NGT组(P<0.05)。GDM组血清镁离子浓度与空腹血糖呈负相关(r=-0.64,P<0.01),25-羟基维生素D3水平与IL-6水平呈负相关(r=-0.57,P<0.01),IL-6水平与空腹血糖呈正相关(r=0.71,P<0.01)。结论孕期血清镁离子,25-羟基维生素D3及IL-6参与GDM血糖水平的调节,维生素D可直接通过炎性途径发挥胰岛素抵抗作用。Objective To investigate the expressions of magnesiumion and,25-hydroxy vitamin D3 and IL-6 in serum of pregnant woman with gestational diabetes mellitus(GDM).Methods A total of 200 pregnant women in the third trimester and single birth pregnant women who received routine antenatal care and delivered in our hospital from October 2018 to October 2019 were enrolled in the study,who were divided into GDM group and normal pregnancy group NGT group,with 100 cases in each group.The serum levels of magnesium,25-hydroxy vitamin D3,IL-6,fasting blood glucose,fasting insulin,insulin resistance index,triglycerides,total cholesterol,high-density lipoprotein cholesterol,and low-density lipoprotein cholesterol were detected and compared between the two groups.Moreover the correlation among serum magnesium,25-hydroxy vitamin D3,IL-6,blood glucose,insulin resistance index,blood lipid levels was analyzed.Results The levels of TG,fasting blood glucose,insulin,and insulin resistance index in GDM group were significantly higher than those in NGT group(P<0.05).The serum levels of magnesium and 25-hydroxy vitamin D3 in GDM group were significantly lower than those in NGT group(P<0.05),however,the serum levels of IL-6 in GDM group were significantly higher than those in NGT group(P<0.05).Moreover the serum levels of magnesium were negatively correlated with those of fasting blood glucose(P<0.01),and the serum levels of 25-hydroxy vitamin D3 were negatively correlated with those of IL-6 level(P<0.01),and IL-6 levels were positively correlated with those of fasting blood glucose(P<0.01).Conclusion The serum levels of magnesium ions,25-hydroxy vitamin D3 and IL-6 may participate in the regulation of blood glucose levels in patients with GMD,and vitamin D can directly exert insulin resistance through inflammatory pathway.
